摘要
目的 观察上皮性卵巢癌患者血清上皮型钙黏蛋白(E-cad)、人附睾蛋白4(HE4)表达情况,并分析二者与术后复发的关系。方法 选取2018年9月—2021年6月于丽水市人民医院接受手术治疗的64例上皮性卵巢癌患者为研究对象,入院时采集患者空腹静脉血3 ml,检测血清E-cad、HE4水平。随访1年,统计患者复发情况,分析血清E-cad、HE4水平与上皮性卵巢癌患者术后复发的关系。结果 随访1年,64例上皮性卵巢癌患者术后复发20例,占31.25%;复发组血清糖类抗原125(CA125)、HE4水平高于未复发组,血清E-cad水平低于未复发组,差异有统计学意义(P<0.05);经点二列相关性分析,结果显示,血清CA125、E-cad、HE4水平与上皮性卵巢癌患者术后复发相关(P<0.05);ROC曲线结果显示,血清E-cad、HE4水平评估上皮性卵巢癌患者术后复发的AUC均>0.8,具有一定评估价值,当E-cad、HE4分别取4.655 pmol/L、218.440 pmol/L时可获得最佳评估价值,其中联合评估价值最高。结论 上皮性卵巢癌患者血清E-cad、HE4水平呈升高表达,二者与术后复发存在密切关系,可用于评估上皮性卵巢癌患者术后复发状况。
Objective This study aims to observe the expression of serum E-cadherin(E-cad) and human epididymal protein 4(HE4) in patients with epithelial ovarian cancer, and to analyze the relationship between them and postoperative recurrence. Methods A total of 64 patients with epithelial ovarian cancer who received surgical treatment in Lishui People’s Hospital from September 2018 to June 2021 were selected as the research subjects. At admission, 3 ml fasting venous blood was collected from patients to detect the levels of serum E-cad and HE4. The patients were followed up for 1 year, the recurrence was counted, and the relationship between the levels of serum E-cad and HE4 and the postoperative recurrence of patients with epithelial ovarian cancer was analyzed. Results After one-year’s follow-up, 20 of 64 patients with epithelial ovarian cancer recurred, accounting for 31.25%;the levels of serum carbohydrate antigen 125(CA125) and HE4 in the recurrence group were higher than those in the non-recurrence group, the level of serum E-cad was lower than that in the non-recurrence group, and the difference was statistically significant(P<0.05);point-biserial correlation analysis showed that the levels of serum CA125, E-cad and HE4 were correlated with postoperative recurrence of patients with epithelial ovarian cancer(P<0.05);ROC curve showed that the AUC of postoperative recurrence in patients with epithelial ovarian cancer were greater than 0.8 in reference with the levels of serum E-cad and HE4, which had certain appraisal value. When E-cad and HE4 were in the concentrations of 4.655 pmol/L and 218.440 pmol/L, the best evaluation value could be obtained, and the joint evaluation value was the highest. Conclusion The levels of serum E-cad and HE4 in patients with epithelial ovarian cancer show an increasing trend. They are closely related to postoperative recurrence, which can be used to evaluate the postoperative recurrence of patients with epithelial ovarian cancer.
