动态心电图及超声血流速度监测对一过性心律失常的诊断价值

摘要 目的:探究动态心电图及超声血流速度监测在一过性心律失常中的诊断价值。方法:选择2020年1月~2022年1月于本院接受治疗的疑似出现一过性心律失常的78例患者为研究对象,分别对其实施常规心电图、动态心电图及超声血流速度监测,以冠脉造影为金标准,判断动态心电图及超声血流速度监测在一过性心律失常中的诊断价值,同时将入组患者按照冠脉造影结果区分为阳性组(44例)和阴性组(34例),对比两组患者动态心电图及超声血流速度指标差异。结果:(1)检测发现,常规心电图对一过性心律失常检测一致性为71.79%,灵敏度为76.19%,特异度为66.67%;(2)动态心电图对一过性心律失常检测一致性为87.18%,灵敏度为86.96%,特异度为87.5%;(3)超声血流速度检测对一过性心律失常检测一致性为89.74%,灵敏度为89.13%,特异度为90.63%;(4)对比显示动态心电图及超声血流速度对一过性心律失常检测一致性、灵敏度、特异度均高于常规心电图(P<0.05);(5)组间对比发现,心律失常阳性组患者Ts-sD、Ts-maxD、Te-masD均高于阴性组患者(P<0.05)。结论:动态心电图及超声血流速度监测在评估一过性心律失常中具有较好的应用价值,同时超声血流速度能够定量分析一过性心律失常患者病情。 Objective: To explore the diagnostic value of dynamic electrocardiogram and ultrasound blood flow velocity monitoring in transient arrhythmia. Methods: 78 patients with suspected transient arrhythmia who were treated in our hospital from January 2020 to January 2022 were selected as the study subjects. They were monitored by routine electrocardiogram, dynamic electrocardiogram and ultrasound blood flow velocity respectively. With coronary angiography as the gold standard, the diagnostic value of dynamic electrocardiogram and ultrasound blood flow velocity monitoring in transient arrhythmia was judged. According to the results of coronary angiography, patients were divided into positive group(44 cases) and negative group(34 cases). The difference of dynamic electrocardiogram and ultrasound blood flow velocity between the two groups was compared. Results:(1) It was found that the consistency, sensitivity and specificity of routine ECG in detecting transient arrhythmia were 71.79%, 76.19% and 66.67% respectively;(2) The consistency, sensitivity and specificity of dynamic electrocardiogram in detecting transient arrhythmia were 87.18%, 86.96% and 87.5% respectively;(3) The consistency, sensitivity and specificity of ultrasonic blood flow velocity detection for transient arrhythmia were 89.74%, 89.13% and 90.63% respectively;(4) The contrast showed that the consistency, sensitivity and specificity of dynamic ECG and ultrasonic blood flow velocity in detecting transient arrhythmia were higher than those of conventional ECG(P<0.05);(5) The comparison between groups showed that the Ts-sD, Ts-maxD and Te-masD of patients with positive arrhythmia were higher than those of patients with negative arrhythmia(P<0.05). Conclusion: Dynamic electrocardiogram and ultrasonic blood flow velocity monitoring have good application value in evaluating transient arrhythmia, and ultrasonic blood flow velocity can quantitatively analyze the condition of patients with transient arrhythmia.
