摘要
目的:观察并分析在2型糖尿病(T2DM)前期及T2DM鉴别中应用全自动生化分析仪的效果。方法:选取2019年5月~2020年6月本院收治的150例糖尿病患者为研究对象,另选取同期150例健康体检者,分别应用全自动生化分析仪与己糖激酶法对300例受检者进行检查与鉴别。结果:(1)全自动生化分析仪与已糖激酶法检测出的T2DM、T1DM患者及健康体检者的空腹血糖、餐后2h血糖水平差异不大,数据对比,差异无统计学意义(P>0.05);(2)全自动生化分析仪在鉴别T2DM及T2DM前期当中具有较高的价值,检验的阳性预测值为97.22%、阴性预测值为92.5%、敏感度为97.22%、特异度为92.5%、准确度为95.95%、约登指数为89.72%、误诊率为7.5%、漏诊率为2.78%;(3)全自动生化分析仪对T2DM及T2DM前期患者进行鉴别时的血液采集量明显比己糖激酶法少,且检验时间明显更少,差异有统计学意义(P>0.05);(4)通过利用全自动生化分析仪鉴别T2DM患者相对于己糖激酶法而言,患者的接受度更好,差异有统计学意义(P<0.05)。结论:全自动生化分析仪应用在T2DM及T2DM前期患者的分别当中诊断价值较高,检测准确率较高,可以节省检验时间,减少血液采集量,患者的接受度更高。
Objective: To observe and analyze the effect of automatic biochemical analyzer in the early stage of type 2 diabetes mellitus(T2DM) and in the identification of T2DM. Methods: A total of 150 patients with diabetes admitted to our hospital from May 2019 to June 2021 were selected as study subjects, and 150 healthy physical examination subjects were selected for the same period. Automatic biochemical analyzer and hexokinase method were used to examine and identify 300 subjects. Results:(1) There was little difference in fasting blood glucose and 2h postprandial blood glucose levels between T2DM and T1DM patients and healthy subjects detected by the automatic biochemical analyzer and the glycokinase method. The difference between the data was not statistically significant(P>0.05);(2) The automatic biochemical analyzer has a high value in the identification of T2DM and the pre-T2DM. The positive predictive value of the test is 97.22%, the negative predictive value is 92.5%, the sensitivity is 97.22%, and the specificity is 92.5%, the accuracy is 95.95%, the Youden index is 89.72%, the misdiagnosis rate is 7.5%, and the missed diagnosis rate is 2.78%;(3)The amount of blood collected by the fully automatic biochemical analyzer for the identification of T2DM and pre-T2DM patients is significantly higher than The hexokinase method is less, and the test time is significantly less, the difference is statistically significant(P>0.05);(4)The acceptance of patients with T2DM compared with the hexokinase method in the identification of patients with T2DM by using an automatic biochemical analyzer Better, the difference is statistically significant(P<0.05). Conclusion: The application of automatic biochemical analyzer in the diagnosis of T2DM and pre-T2DM patients has a higher diagnostic value, a higher detection accuracy rate, which can save test time, reduce the amount of blood drawn, and is more acceptable to patients.
