摘要
目的探讨L_(2)和L_(5)交感神经分支在椎间盘源性腰痛传导通路中的作用。方法将120只SD大鼠分为椎间盘前部组(A组,n=60)和椎间盘后部组(B组,n=60),再根据交感神经分支切断部位将A组分为A0(未切断交感神经分支)、A-L_(2)、A-L_(5)、A-L_(2)-L_(5)组,各15只大鼠;将B组分为B0(未切断交感神经分支)、B-L_(2)、B-L_(5)、B-L_(2)-L_(5)组,各15只大鼠。记录T_(13)、L_(1)、L_(2)、L_(3)、L_(4)、L_(5)和L6区段中,在大鼠背根神经节(DRG)处用荧光金(FG)和物质P(SP)双标记的细胞数量。结果A-L_(2)和A-L_(2)-L_(5)组在L_(2)区段带有双标记的DRG细胞数量明显少于A0组(P<0.01)。与B0组相比,L_(2)区段中B-L_(2)组带有双标记的DRG细胞数量明显更少(P<0.01);在T_(13)、L_(1)和L_(2)区段中,B-L_(5)组带有双标记的DRG细胞数量明显更少(P<0.01),T_(13)、L_(1)和L_(2)区段中,B-L_(2)-L_(5)组带有双标记的DRG细胞数量明显更少(P<0.01)。L_(2)区段中,B-L_(2)-L_(5)组带有双标记的DRG细胞数量明显少于B-L_(2)组和B-L_(5)组(P<0.01)。结论在大鼠中,L_(2)交感神经分支参与从L_(5)~6椎间盘至L_(2)背根神经节的疼痛信息传导,而来自L_(5)~6椎间盘的疼痛信息通过两种不同途径由上下腰椎背根神经节介导。L_(5)交感神经分支介导了上腰椎背根神经节的传导通路。
Objective To investigate the effect of the L_(2) and L_(5) sympathetic nerve branches in the conduction pathway of discogenic low back pain.Methods 120 SD rats were divided into anterior intervertebral disc group(group A,n=60)and posterior intervertebral disc group(group B,n=60),and then group A was divided into A0(without cutting off the sympathetic nerve branch),A-L_(2),A-L_(5),A-L_(2)-L_(5) groups,15 rats each,according to the cutting position of the sympathetic nerve branch;group B was divided into B0(without cutting off the sympathetic branch),B-L_(2),B-L_(5) and B-L_(2)-L_(5) groups,15 rats each.The numbers of fluorogold(FG)and Substance P(SP)double-labeled cells in the DRGs of the T_(13),L_(1),L_(2),L_(3),L_(4),L_(5) and L6 segments were recorded.Results The numbers of double-labeled DRG cells in the L_(2) segment were significantly lower in the A-L_(2) and A-L_(2)-L_(5) groups than that in the group A0(P<0.01).Compared to the group B0,the B-L_(2) group had significantly fewer double-labeled DRG cells in the L_(2)segment(P<0.01);the B-L_(5) group had significantly fewer double-labeled DRG cells in the T_(13),L_(1) and L_(2) segments(P<0.01)and the B-L_(2)-L_(5) group had significantly fewer double-labeled DRG cells in the T_(13),L_(1) and L_(2) segments(P<0.01).The numbers of double-labeled DRG cells in the L_(2) segment in the B-L_(2)-L_(5) group were significantly lower than those in the B-L_(2) and B-L_(5) groups(P<0.01).Conclusion In rats,the L_(2) sympathetic nerve branch participates in conduction of pain signal from the L__(5-6) intervertebral disc to the L_(2) DRG.The pain signal from the L_(5-6) intervertebral disc is conducted to the upper and lower lumbar vertebrae DRGs through two different pathways.The L_(5) sympathetic nerve branches mediate the conduction pathway of the upper lumbar vertebra DRG.
