Influencing factors of β receptor blocker medication compliance in AMI patients after PCI

目的:研究急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后β受体阻滞剂用药依从性降低的影响因素。方法:依据8项Morisky药物依从性量表评分,于我院行PCI术的130例AMI患者被分为依从性良好组(55例)与依从性差组(75例)。比较两组一般临床资料,分析AMI患者PCI术后β受体阻滞剂用药依从性降低的影响因素。结果:与依从性良好组比较,依从性差组年龄≥60岁、初中文化程度、月平均收入<6000元、医疗费用自费、糖尿病、心率≤70次/min比例均显著升高(P<0.05或<0.01)。多因素Logistic回归分析显示,年龄≥60岁、初中文化程度、月平均收入<6000元、医疗费用自费、糖尿病、心率≤70次/min是AMI患者PCI术后β受体阻滞剂用药依从性降低的独立危险因素(OR=3.074~94.861,P<0.05或<0.01)。结论:急性心肌梗死患者PCI术后β受体阻滞剂用药依从性受年龄、文化程度、收入、心率等多项因素影响。

Objective: To study influencing factors of reduced β receptor blocker medication compliance in patients with acute myocardial infarction(AMI) after percutaneous coronary intervention(PCI). Methods:According to score of 8-item Morisky medication adherence scale, a total of 130 AMI patients undergoing PCI in our hospital were divided into good compliance group(n=55) and poor compliance group(n=75). General clinical data were compared between two groups. Influencing factors of reduced β receptor blocker medication compliance in AMI patients after PCI were analyzed. Results:Compared with good compliance group, there were significant rise in percentages of age≥60 years old, education level of middle school, monthly average income<6000 RMB, self-paying medical expenses, diabetes mellitus(DM) and heart rate(HR)≤70 beats/min in poor compliance group(P<0.05 or <0.01). Multivariate Logistic regression analysis indicated that age≥60 years old, education level of middle school, monthly average income<6000 RMB, self-paying medical expenses, DM and HR≤70 beats/min were independent risk factors for reduced medication compliance of β receptor blocker in AMI patients after PCI(OR=3.074~94.861, P<0.05 or <0.01). Conclusion: β receptor blocker medication compliance is affected by age, education, income, HR and other factors in patients with acute myocardial infarction after PCI.
