摘要
目的:研究新疆维吾尔族高血压合并冠心病患者冠脉狭窄程度与糖尿病和脂蛋白(a)[Lp(a)]的关系。方法:依据冠脉造影,我院的368例维吾尔族高血压患者被分为冠心病组(259例)和非冠心病组(109例)。比较两组一般临床资料,分析冠心病的影响因素,其危险因素与新疆维吾尔族高血压合并冠心病患者Gensini评分的关系。结果:与非冠心病组比较,冠心病组年龄、男性、吸烟史、糖尿病比例,血清糖化血红蛋白(HbA1c)水平均显著升高(P<0.05或<0.01)。多因素Logistic回归分析显示,男性、年龄、Lp(a)、HbA1c为冠心病的独立危险因素(OR=1.001~4.204,P<0.05或<0.01)。多元线性回归分析显示,糖尿病与新疆维吾尔族高血压合并冠心病患者的Gensini评分正相关(B=6.758,P=0.037)。结论:男性、年龄、Lp(a)、HbA1c是维吾尔族高血压患者发生冠心病的危险因素,且糖尿病与新疆维吾尔族高血压合并冠心病患者冠脉狭窄程度呈正相关。
Objective:To study relationship among diabetes mellitus, Lp(a) and coronary stenosis severity in hypertension complicated coronary heart disease(CHD) patients of Xinjiang Uygur. Methods: According to coronary angiography, a total of 368 Uygur patients with hypertension treated in our hospital were divided into CHD group(n=259) and no CHD group(n=109). General clinical data were compared between two groups. Influencing factors of CHD and relationship between CHD risk factors and Gensini score in Xinjiang Uygur patients with hypertension complicated CHD were analyzed. Results: Compared with no CHD group, there were significant rise in age, percentages of male, smoking history and diabetes mellitus(DM) and serum HbA1 c level in CHD group(P<0.05 or <0.01). Multivariate Logistic regression analysis indicated that male, age, Lp(a) and HbA1 c were independent risk factors for CHD(OR=1.001~4.204, P<0.05 or <0.01). Multiple linear regression analysis indicated that DM is positively correlated with Gensini score in Xinjiang Uygur patients with hypertension complicated CHD(B=6.758, P=0.037).Conclusion: Male, age, Lp(a) and HbA1 c are risk factors for coronary heart disease in Uygur hypertensive patients. DM is positively correlated with Gensini score in Xinjiang Uygur patients with hypertension complicated CHD.
