摘要
目的探讨三维盆底超声在产后早期盆底功能障碍诊断中的临床应用价值。方法选取产后早期有盆底功能障碍者102例为观察组,选取同期分娩但产后无盆底功能障碍者100例为对照组,所有患者均采用彩色多普勒超声诊断。比较两组在静息、缩肛、最大Valsalva状态下相关参数及肛提肌肌力情况。结果静息状态下,两组膀胱颈位置、宫颈外口位置、尿道倾斜角比较,差异均有统计学意义(P<0.05);Valsalva状态下,观察组膀胱颈移动度、尿道旋转角大于对照组,膀胱后角开放例数多于对照组,阴道气体线长度短于对照组,差异均有统计学意义(P<0.05)。观察组肛提肌肌力情况优于对照组,差异有统计学意义(P<0.05)。结论三维盆底超声在产后早期盆底功能障碍诊断中应用价值较高,为临床早期发现、及时治疗提供可靠参考,值得临床推广与应用。
Objective To explore the clinical value of three-dimensional pelvic floor ultrasound in the diagnosis of early postpartum pelvic floor dysfunction. Methods A total of 102 postpartum women with pelvic floor dysfunction in the early postpartum period were selected as the observation group, and 100 postpartum women without pelvic floor dysfunction in the same period were selected as the control group. All patients were diagnosed by color Doppler ultrasound. The relevant parameters and levator ANI muscle strength were compared between the two groups at rest, during anal contraction and maximum Valsalva.Results In the resting state, there were significant differences in bladder neck position, cervical external orifice position and urethral inclination Angle between the two groups(P<0.05). Under Valsalva, the bladder neck mobility and urethral rotation Angle of the observation group were higher than those of the control group, the number of cases with open posterior bladder Angle was more than that of the control group, and the vaginal gas line length was shorter than that of the control group, and the differences were statistically significant(P<0.05). The levator ANI muscle strength of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05). Conclusion Three-dimensional pelvic floor ultrasound has a high value in the diagnosis of early postpartum pelvic floor dysfunction, which provides a reliable reference for early clinical detection and timely treatment, and is worthy of clinical promotion and application.
