分娩镇痛对产妇产程中宫缩及产后尿潴留的影响

Effect of Labor Analgesia on Midlabor Contraction and Postpartum Urinary Retention

目的:探讨分娩镇痛对产妇产程中宫缩及产后尿潴留的影响。方法:选取2020年5月-2021年10月北京市门头沟区妇幼保健院妇产科收治的无妊娠合并症、单胎、足月临产后行硬膜外阻滞阴道分娩的产妇370例为观察组;选取同期无妊娠合并症、单胎、足月临产未采取任何分娩镇痛分娩的产妇100例为对照组。比较两组产妇分娩疼痛程度,宫缩指标,产后6h后尿潴留发生率。结果:观察组分娩疼痛程度低于对照组,差异有统计学意义(P<0.05);两组产妇第一产程与第二产程中宫缩压力、宫缩间隔时间、宫缩持续时间比较,差异无统计学意义(P>0.05);两组产妇产后6h尿潴留发生率比较,差异无统计学意义(P>0.05)。结论:分娩镇痛可有效降低产程中疼痛程度,对产程中宫缩强度无明显影响,未缩短宫缩持续时间或降低宫缩压力,不增加产后尿潴留发生率。

Objective:To investigate the effect of labor analgesia on contractions during labor and postpartum urinary retention.Methods:From May 2020 to October 2021,370 pregnant women who admitted to department of obstetrics and gynecology of Mentougou Maternal and Child Health Hospital of Beijing without pregnancy complications,single pregnancy and full term delivery with epidural block for vaginal delivery were selected as the observation group.100 maternal patients with no pregnancy complications,single pregnancy and full term delivery without any labor analgesia were selected as the control group.The degree of labor pain,uterine index and incidence of urine retention 6h after delivery were compared between the two groups.Results:The degree of labor pain in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in uterine pressure,uterine interval time and uterine duration between the two groups(P>0.05).There was no significant difference in the incidence of urine retention at 6h postpartum between the two groups(P>0.05).Conclusion:Labor analgesia can effectively reduce the degree of pain during labor,but has no significant effect on the intensity of contractions during labor,does not shorten the duration of contractions or reduce the pressure of contractions,and does not increase the incidence of postpartum urinary retention.
