个案管理模式在血液透析患者中的应用价值

Application Value of Case Management Model in Hemodialysis Patients

目的:分析个案管理模式在血液透析患者中的应用价值。方法:选取2021年4月-2022年1月肇庆市第二人民医院肾内科收治的慢性肾功能不全尿毒症期的血液透析患者100例为研究对象,按照随机数字表法分为对照组与观察组,各50例。对照组给予常规护理干预,观察组给予个案管理模式护理干预。比较两组血清学指标、有效干体重知晓率、再入院率、生活质量、护理满意度。结果:护理后,两组患者血钾、血钙、血磷、血肌酐水平均降低,血红蛋白水平均升高,观察组血钾、血钙、血磷、血肌酐水平低于对照组,血红蛋白高于对照组,差异有统计学意义(P<0.05);护理后,两组患者干体重知晓率、生活质量评分均升高,观察组高于对照组,差异有统计学意义(P<0.05);观察组再入院率低于对照组,差异有统计学意义(P<0.05);观察组护理满意度高于对照组,差异有统计学意义(P<0.05)。结论:个案管理模式可改善血液透析患者血清学指标及健康知识知晓情况,降低再入院率,提高生活质量及护理满意度。

Objective:To analyze the application value of case management model in hemodialysis patients.Methods:A total of 100 hemodialysis patients with uremia with chronic renal insufficiency admitted to department of nephrology of Zhaoqing Second People's Hospital from April 2021 to January 2022 were selected as the research objects,and were divided into control group and observation group according to random number table method,with 50 cases in each group.The control group was given routine nursing intervention,and the observation group was given case management mode nursing intervention.Serological indexes,effective dry weight awareness rate,readmission rate,quality of life and nursing satisfaction were compared between the two groups.Results:After nursing,the levels of blood potassium,blood calcium,blood phosphorus and blood creatinine in 2 groups were decreased,while hemoglobin levels were increased.The levels of blood potassium,blood calcium,blood phosphorus and blood creatinine in the observation group were lower than those in the control group,while hemoglobin was higher than that in the control group,the difference was statistically significant(P<0.05).After nursing,dry weight awareness rate and quality of life score of patients in 2 groups were increased,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).The readmission rate of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Nursing satisfaction of observation group was higher than control group,the difference was statistically significant(P<0.05).Conclusion:The case management model can improve the serological index and health knowledge of hemodialysis patients,reduce the readmission rate,improve the quality of life and nursing satisfaction.
