PDCA在全身麻醉期间体温管理中的应用

摘要 目的:探究PDCA在全身麻醉期间体温管理中的应用效果。方法:回顾性分析2020年8月-2022年3月于南昌大学第一附属医院行全身麻醉的47560例患者的临床资料。将2020年8月-2021年5月全身麻醉期间实施常规管理的23780例患者设为对照组,2021年6月-2022年3月全身麻醉期间实施PDCA循环管理的23780例患者设为观察组。比较两组低体温发生率、术后恢复情况、核心体温变化情况、术后并发症发生率。结果:与对照组相比,观察组低体温发生率较低,麻醉苏醒时间较短,差异均有统计学意义(P<0.05);对比两组入室时核心体温,差异无统计学意义(P>0.05);观察组麻醉后0.5、1 h、手术结束时核心体温均高于对照组,差异均有统计学意义(P<0.05);与对照组并发症发生率相比,观察组较低,差异有统计学意义(P<0.05)。结论:PDCA循环管理能够有效维持全身麻醉期间患者体温稳定,预防低体温的发生,降低术后并发症发生风险,加强术中体温监测和处理,有助于改善患者预后、促进术后快速康复。 Objective:To explore the application effect of PDCA in body temperature management during general anesthesia.Method:A retrospective analysis of the clinical data of 47560 patients undergoing general anesthesia in the First Affiliated Hospital of Nanchang University from August 2020 to March 2022,and 23780 patients who received routine management during general anesthesia from August 2020 to May 2021 were set as the control group,and 23780 patients who received PDCA cycle management during general anesthesia from June 2021 to March 2022 were set as the observation group.The incidence of hypothermia,postoperative recovery,changes in core body temperature and postoperative complications were compared between the two groups.Result:Compared with the control group,the incidence of hypothermia was lower in the observation group,and the recovery time after anesthesia was shorter,the differences were statistically significant(P<0.05).There was no significant difference in core body temperature between the two groups(P>0.05).The core body temperature of the observation group was higher than that of the control group at 0.5,1 h after anesthesia and at the end of operation,with statistical significance(P<0.05).Compared with the control group,the incidence of complications in the observation group was lower,the difference was statistically significant(P<0.05).Conclusion:PDCA cycle management can effectively maintain the body temperature of patients during general anesthesia,prevent the occurrence of hypothermia,reduce the risk of postoperative complications,and strengthen the monitoring and treatment of intraoperative temperature,which can help improve the prognosis of patients and promote rapid postoperative recovery.
