CT与磁共振成像诊断儿童中枢神经系统神经母细胞瘤的价值

The Value of CT and MRI in the Diagnosis of Neuroblastoma of the Central Nervous System in Children

目的:研究CT与磁共振成像诊断儿童中枢神经系统神经母细胞瘤的价值。方法:选入2021年1月-2022年1月于郑州大学附属儿童医院进行检查的49例高度疑似中枢神经系统神经母细胞瘤儿童作为本文的研究对象,49例儿童都进行CT和磁共振成像检查。以病理组织学检查结果为金标准,对比CT、磁共振成像诊断儿童中枢神经系统神经母细胞瘤结果、诊断价值分析、影像学特点。结果:病理诊断43例中枢神经系统神经母细胞瘤,CT诊断35例,磁共振成像诊断41例。磁共振成像诊断的敏感度、特异度、准确率、阳性预测值、阴性预测值都比CT诊断高,其中敏感度、准确率差异均有统计学意义(P<0.05);CT平扫儿童左侧顶叶可见囊实性肿块;磁共振成像检查能够看见儿童脑桥位置存在病灶,且已经出现肿胀情况。结论:CT与磁共振成像在儿童中枢神经系统神经母细胞瘤的影像学表现具有一定的特征,磁共振成像的诊断价值高于CT,但是儿童中枢神经系统神经母细胞瘤临床诊断治疗仍然需要进行病理检查。

Objective:To study the value of CT and magnetic resonance imaging in the diagnosis of neuroblastoma of the central nervous system in children.Method:A total of 49 children with highly suspected neuroblastoma of the central nervous system who were examined in Children's Hospital Affiliated to Zhengzhou University from January 2021 to January 2022 were selected as the subjects of this study,CT and MRI were performed in 49 children.The results of histopathology were taken as the gold standard.The results of CT diagnosis and MRI diagnosis of children's central nervous system neuroblastoma,the diagnostic value of CT and MRI,and the imaging characteristics of CT and MRI were compared.Result:Pathological diagnosis included 43 cases of neuroblastoma of central nervous system,35 cases of CT and 41 cases of MRI.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of MRI diagnosis were higher than those of CT diagnosis,the differences in sensitivity and accuracy were statistically significant(P<0.05).On CT plain scan,cystic and solid masses could be seen in the left parietal lobe of children;magnetic resonance imaging could see that there were lesions in the pons of children,and swelling had occurred.Conclusion:CT and MRI have certain characteristics in the imaging manifestations of neuroblastoma of the central nervous system in children.The diagnostic value of MRI is higher than that of CT.However,the clinical diagnosis and treatment of neuroblastoma of the central nervous system in children still need pathological examination.
