宫颈感染HPV患者相关免疫指标水平的变化及意义 被引量:2

Changes and significance of the relevant immune index levels in patients with cervical HPV infection

摘要 目的分析宫颈感染人乳头瘤病毒(Human papillomavirus,HPV)患者免疫指标水平的变化及意义。方法根据HPV不同型别将2018年2月至2020年1月就诊于长沙市妇幼保健院妇产科的300例宫颈感染HPV患者分为高危型组(128例)和低危型组(172例)。另选150例健康女性作为对照组。比较3组T淋巴亚群细胞因子、Th1/Th2型细胞因子水平,分析相关性。结果高危型组CD3+、CD3+CD4+、CD4+/CD8+、CD3-CD56+及IFN-γ、IL-2水平<低危型组<对照组,CD3+CD8+、CD19+及IL-4水平>低危型组>对照组(P<0.05);CD3+、CD3+CD4+、CD4+/CD8+、CD3-CD56+与IFN-γ、IL-2呈正相关(r>0,P<0.05),与IL-4呈负相关(r<0,P<0.05);CD3+CD8+、CD19+水平与IFN-γ、IL-2呈负相关(r<0,P<0.05),与IL-4呈正相关(r>0,P<0.05)。结论宫颈感染HPV患者免疫指标水平与炎症反应密切相关,且高危型HPV患者免疫系统紊乱,且免疫功能低下。 Objective To analyze the change and significance of relevant immune index levels among patients with cervical human papillomavirus(HPV)infection.Methods Three hundred patients with cervical HPV infection in Obstetrics and Gynecology Department of Changsha Maternal and Child Health Hospital from February 2018 to January 2020 were divided into the high-risk group(128 cases)and the low-risk group(172 cases)according to the types of HPV.Another 150 healthy women were selected as the control group.T lymphocyte subsets cytokines,immunoglobulins(IgA,IgG and IgM)and Th1/Th2 cytokines(IFN)were compared and the correlation between immune indexes and Th1/Th2 cytokines was analyzed.Results For the levels of CD3+,CD3+CD4+,CD4+/CD8+,CD3-CD56+,IgA,IgG,IFN-γand IL-2,high-risk group was the lowest.Followed by low-risk group and control group and the levels of CD3+CD8+,CD19+and IL-4 in high-risk group were the highest followed by low-risk group and control group(P<0.05).The levels of CD3+,CD3+CD4+,CD4+/CD8+,CD3-CD56+were positively correlated to levels of IFN-γ,and IL-2(r>0,P<0.05)and negatively correlated with IL-4(r<0,P<0.05).The levels of CD3+CD8+and CD19+were negatively correlated to IFN-γand IL-2(r<0,P<0.05)and positively correlated with IL-4(r>0,P<0.05).Conclusions The level of immune indexes in patients with cervical infection of HPV was closely related to inflammatory response,and the patients infected with high-risk HPV showed immune dysfunction and low response.
