抗精神病药物对ICU谵妄患者治疗效果的研究进展

摘要 谵妄是一种急性、可逆性和广泛性的意识障碍,危重症患者是谵妄的高发人群。重症监护病房(intensive care unit,ICU)谵妄严重影响患者的远期转归,其不良后果包括ICU住院时间延长、额外的医疗费用增加、术后并发症和病死率增加等。近年来,ICU谵妄的临床识别率逐渐提高,但目前学术界对其发病机制和治疗方法尚无统一认识。虽然谵妄的病理生理机制还没有明确,但公认较多的是中枢神经递质的失衡。本文就谵妄的定义与分型、病理生理机制、抗精神病药物的药理特性和临床应用情况进行阐述,旨在从抗精神病药物调节中枢神经系统功能出发,为ICU谵妄的治疗提供一定的临床依据。
