LEARNS模式在腹腔镜前列腺癌根治术后患者尿失禁健康教育中的应用 被引量:2

Effects of LEARNS model in health education of urinary incontinence for patients undergoing laparoscopic radical prostatectomy

摘要 目的探讨LEARNS模式在腹腔镜前列腺癌根治术后尿失禁健康教育中的应用效果。方法于2019年9月—2020年8月便利选择北京大学第一医院泌尿外科行腹腔镜前列腺癌根治术的60例患者,按照入院时间先后分为对照组和观察组,各30例。对照组给予常规教育模式;观察组实施LEARNS模式,具体步骤包括聆听(listen,L)、建立(establish,E)、应用(adopt,A)、提高(reinforce,R)、反馈教学(name,N)、强化(strengthen,S)。比较两组患者出院前的盆底肌训练掌握情况,观察两组患者出院1、3个月的控尿情况。结果观察组患者的盆底肌训练知识水平及正确性均高于对照组,差异均有统计学意义(P<0.01)。术后3个月,观察组患者的尿失禁得分低于对照组,尿垫使用个数少于对照组,控尿率高于对照组,差异均有统计学意义(P<0.01)。结论LEARNS模式能有效提高腹腔镜前列腺癌根治术患者的盆底肌训练掌握情况,改善患者术后控尿情况。 Objective To explore the effects of the LEARNS model in health education of urinary incontinence for patients undergoing laparoscopic radical prostatectomy.Methods From September 2019 to August 2020,totally 60 patients who underwent laparoscopic radical prostatectomy in the Department of Urinary Surgery,Peking University First Hospital were selected by convenience sampling and divided into a control group and an intervention group according to the time of admission,with 30 cases in each group.Patients in the control group underwent conventional health education,while patients in the intervention group received nursing using the LEARNS model,with specific steps including listen(L),establish(E),adopt(A),reinforce(R),name(N),and strengthen(S).The mastery of knowledge about pelvic floor muscle before discharge was compared between the two groups,and the urinary continence of the two groups at 1 and 3 months after discharge was observed.Results The knowledge level and correctness of pelvic floor muscle in the intervention group were higher than those in the control group,and the dferences were statistically significant(P<0.01).At 3 months after surgery,the urinary incontinence score of the intervention group was lower than that of the control group;the number of urine pads used was less than that of the control group;the rate of continence was higher than that of the control group,with statistically significant differences(P<0.01).Conclusions The LEARNS mode can effectively improve the mastery of knowledge about pelvic floor muscle in patients undergoing laparoscopic radical prostatectomy,and improve their postoperative urinary continence.
