慢性肺部疾病患者健康相关生命质量及其影响因素研究

Health-related Quality of Life and Its Associated Factors among Patients with Chronic Lung Disease

目的:测量我国中老年慢性肺部疾病患者的健康相关生命质量,并探索其影响因素,提出改善对策。方法:数据来源于2018年中国健康与养老追踪调查(CHARLS)中728名慢性肺部疾病患者,基于欧洲五维度三水平生命质量量表(EQ-5D-3L)计算健康效用值以反映其健康相关生命质量水平。采用Mann-Whitney或Kruskal-Wallis检验比较健康效用值的组间差异;构建Tobit回归模型分析健康相关生命质量的影响因素。结果:慢性肺部疾病患者健康效用值的中位数为0.779(四分位数间距为0.232),女性、高龄、夜间睡眠不足、共患其他慢性病、无中等强度体力活动者的健康相关生命质量均较低,差异有统计学意义(P<0.05)。结论:慢性肺部疾病患者的健康相关生命质量较低,影响因素复杂。建议社会多方协作,采取整合性的措施,提升患者健康相关生命质量。

Objective:To measure the health-related quality of life(HRQoL)of middle-aged and elderly patients with chronic lung disease in China, and explore its associated factors, then put forward improvement measures.Methods:Data were obtained from 728 patients in the 2018 China Health and Retirement Longitudinal Study(CHARLS).The 3-level version of the EuroQoL Five Dimensions Questionnaire(EQ-5D-3L)was used to calculate the health utility value of patients to indicate the level of HRQoL.Mann-Whitney or Kruskal-Wallis test was used to compare between-group differences in health utility values.Tobit regression model was used to analyze the factors influencing HRQoL.Results:The median health utility value for patients with chronic lung disease was 0.779(interquartile range was 0.232),and the level of HRQoL was statistically lower for women, elderly, lack of sleep at night, comorbidity with other chronic diseases, and no moderate physical activity(P<0.05).Conclusion:The HRQoL of patients with chronic lung disease is relatively low, and its associated factors are complex.It is recommended that multi-parts of society collaborates on integrated measures to improve the HRQoL of patients.
