扶正解毒方对高危型HPV感染患者HPV转阴率及中医证候积分的影响

Effect of Fuzheng Jiedu Prescription on HPV Negative Conversion Rate and TCM Syndrome Score in Patients with High-risk HPV Infection

目的探究扶正解毒方对高危型人乳头瘤病毒(high risk human papillomavirus,HPV)患者HPV转阴率及中医症候积分的影响。方法选择福建医科大学附属三明第一医院2015年9月—2019年12月收治的200例高危型HPV感染患者为探究对象,根据随机数表法将入选患者分为联合组及西药组。西药组(n=100例)行银尔舒治疗,联合组(n=100例)在西药组的基础上应用扶正解毒方治疗。观察两组患者的总有效率、复发率及HPV转阴率,应用中医证候积分量表评价两组患者的中医证候积分,比较两组患者治疗前后的病毒负荷量、炎症因子水平。结果西药组的总有效率90.00%低于联合组患者的98.00%,差异有统计学意义(χ^(2)=5.674,P=0.017)。治疗后,两组患者IL-2水平较治疗前呈现显著增高趋势,病毒负荷量、IL-4、hs-CRP水平较治疗前呈现降低趋势,且联合组患者的上述指标均优于西药组,差异有统计学意义(P<0.05)。治疗后,中医症候积分呈现明显降低趋势,观察组患者的中医证候积分(2.41±0.61)分,明显低于西药组患者的(5.73±0.68)分,差异有统计学意义(t=32.403,P<0.05)。西药组的HPV转阴率为75.00%低于联合组的95.00%,差异有统计学意义(χ^(2)=15.686,P<0.05)。结论扶正解毒方能有效提高高危型HPV感染患者的整体效率,降低中医证候积分,病毒负荷量相应减少,炎症反应得到抑制,临床应用有效,促进患者HPV转阴。

Objective To explore the effect of Fuzheng Jiedu Fang on HPV negative conversion rate and TCM symptom score in high-risk human papillomavirus(HPV)patients.Methods A total of 200 patients with high-risk HPV infection who were admitted to Sanming First Hospital Affiliated to Fujian Medical University from September 2015 to December 2019 were selected as the research objects.According to the random number table method,the selected patients were divided into the combination group and the western medicine group.The western medicine group(n=100cases)was treated with Yinershu,and the combined group(n=100 cases)was treated with Fuzheng Jiedu prescription on the basis of the western medicine group.The total effective rate,recurrence rate and HPV negative rate of the two groups were observed.The TCM syndrome scores of the two groups were evaluated by the TCM syndrome score scale.The viral load and inflammatory factor levels were compared between the two groups before and after treatment.Results The total effective rate of 90.00%in the western medicine group was lower than that of 98.00%in the combined group,and the difference was statistically significant(χ^(2)=5.674,P=0.017).After treatment,the levels of IL-2 in the two groups were significantly increased compared with those before treatment,and the levels of viral load,IL-4 and hs-CRP were decreased compared with those before treatment.And the above indexes of the patients in the combination group were better than those in the western medicine group,and the difference was statistically significant(P<0.05).After treatment,the TCM syndrome score showed a significant decreasing trend.The TCM syndrome score of the observation group was(2.41±0.61)points,which was significantly lower than that of the western medicine group(5.73±0.68)points,and the difference was statistically significant(t=32.403,P<0.05).The negative rate of HPV in western medicine group was 75.00%,which was lower than 95.00%in combination group,and the difference was statistically significant(χ^(2)=15.686,P<0.05).Conclusion Fuzheng Jiedu prescription can effectively improve the overall efficiency of patients with high-risk HPV infection,reduce TCM syndrome scores,correspondingly reduce viral load,inhibit inflammatory response,and has effective clinical application to promote HPV negative conversion in patients.
