乙型肝炎肝硬化患者HBV-DNA表达与血小板参数及D-二聚体的相关性

Correlation Between HBV-DNA Expression and Platelet Parameters and D-dimer in Patients with Hepatitis B Cirrhosis

目的:分析乙型肝炎肝硬化患者乙型肝炎病毒DNA(HBV-DNA)表达与血小板参数、D-二聚体(D-D)的相关性。方法:便利抽取80例于2019年1月-2022年1月在宜春市人民医院接受治疗的乙型肝炎肝硬化患者作为观察组,并予以HBV-DNA检查,根据HBV-DNA载量分为低、中、高载毒量组,另随机选取25例健康人(同期在本院进行常规体检)作为对照组,所有研究对象均进行血小板参数[血小板计数(PLT)、血小板体积分布宽度(PDW)及平均血小板体积(MPV)]、D-D检查。对比观察组、对照组及不同HBV-DNA载毒量组之间的血小板参数、D-D水平的差异性,并采用Pearson相关性分析探究乙型肝炎肝硬化患者HBV-DNA表达与血小板参数、D-D的相关性。结果:对比观察组和对照组血小板参数、D-D水平,观察组PLT水平低于对照组,MPV、PDW、D-D水平均高于对照组(P<0.05);对比三组不同载毒量组血小板参数、D-D水平,差异均存在统计学意义(P<0.05);Pearson分析结果显示乙型肝炎肝硬化患者HBV-DNA表达与PLT呈负相关,与MPV、PDW及D-D水平均呈正相关(P<0.05)。结论:乙型肝炎肝硬化患者HBV-DNA载量与血小板参数及D-D存在相关性,患者HBV-DNA载量越高,其凝血纤溶功能失衡更明显。

Objective: To analyze the correlation between hepatitis B virus DNA(HBV-DNA) expression and platelet parameters and D-dimer(D-D) in patients with hepatitis B cirrhosis. Method: A total of 80 patients with hepatitis B cirrhosis who were treated in Yichun People’s Hospital from January 2019 to January 2022 were conveniently selected as the observation group and subjected to HBV-DNA examination. They were divided into low,medium and high drug loading groups according to HBV-DNA load. In addition, 25 healthy people(who underwent routine physical examination in our hospital at the same time) were randomly selected as the control group. Platelet parameters [platelet count(PLT), platelet distribution width(PDW) and mean platelet volume(MPV)] and D-D were examined in all the subjects. The differences of platelet parameters and D-D levels among observation group,control group and different HBV-DNA loading groups were compared, and Pearson correlation analysis was used to explore the correlation between HBV-DNA expression and platelet parameters and D-D in patients with hepatitis B cirrhosis. Result: Comparing the platelet parameters and D-D levels between the observation group and the control group, the PLT level in the observation group was lower than that in the control group, while the MPV, PDW and D-D levels were higher than those in the control group(P<0.05). Comparing the platelet parameters and D-D levels of three groups with different drug loading, the differences were statistically significant(P<0.05). Pearson analysis showed that HBV-DNA expression was negatively correlated with PLT, and positively correlated with MPV, PDW and D-D levels(P<0.05). Conclusion: HBV-DNA loading in patients with hepatitis B cirrhosis is correlated with platelet parameters and D-D. The higher the HBV-DNA loading, the more obvious the imbalance of coagulation and fibrinolysis function.
