直肠癌患者低位前切除术后不同引流入路的应用效果对比

Comparison of effect of different drainage approaches after low anterior resection on patients with rectal cancer

目的探讨直肠癌患者低位前切除术后不同引流入路的应用效果。方法抽取2018年4月至2021年4月洛阳东方人民医院收治的直肠癌患者120例,采用随机数字表法分为A组与B组,每组60例。两组均行低位前切除术治疗,A组术后选择经会阴引流,B组术后选择经腹壁引流。比较两组引流管管周情况(引流管切口分泌物消失时间、引流管管周红肿消退与疼痛消失时间)、引流状况(引流物持续时间、引流物总量与拔除引流管时间)与术后恢复指标(下床活动、胃肠排气与住院时间),并比较两组术后并发症发生率。结果A组引流管管周疼痛消失、引流管切口分泌物消失与红肿消退时间均短于B组(P均<0.05);A组引流物持续时间、拔除引流管时间均短于B组,引流物总量多于B组(P<0.05);A组住院时间短于B组(P<0.05);两组下床活动时间与胃肠排气时间比较,差异未见统计学意义(P>0.05);A组并发症发生率(5.00%,3/60)低于B组(16.67%,10/60),P<0.05。结论与经腹壁引流比较,低位前切除术后经会阴引流的引流效果更佳,可缩短直肠癌术后患者住院时间、引流管管周恢复时间,降低患者术后并发症发生率。

Objective To investigate the application effect of different drainage approaches after low anterior resection on patients with rectal cancer.Methods A total of 120 patients with rectal cancer treated in Luoyang Dongfang People’s Hospital from April 2018 to April 2021 were selected.And they patients were divided into group A and group B by random number table method,with 60 cases in each group.Both groups were treated by low anterior resection;after operation,the group A was treated by perineal drainage,and the group B was treated by abdominal drainage.The two groups were compared in terms of peridrainage conditions(disappearance time of incision secretion of drainage tube,regression time of redness and swelling,and disappearance time of pain around drainage tube),drainage characteristics(duration of drainage,total amount of drainage and removal time of drainage tube)and postoperative recovery indexes(out of bed activity time,gastrointestinal exhaust time and length of hospital stay);and the incidence of postoperative complications were compared between the two groups.Results The disappearance time of pain around drainage tube,disappearance time of incision secretion of drainage tube,regression time of redness and swelling in the group A were shorter than those in the group B(all P<0.05);the duration of drainage and time of removing drainage tube in the group A were shorter than those in the group B,and the total amount of drainage was more than that in the group B(P<0.05);the length of hospital stay in the group A was short than that in the group B(P<0.05);there was no significant difference in the time of getting out of bed and gastrointestinal exhaust between the two groups(P>0.05);the complication rate of the group A(5.00%,3/60)was lower than that of the group B(16.67%,10/60),P<0.05.Conclusions Compared with abdominal wall drainage,perineal drainage after low anterior resection in patients with rectal cancer can shorten the time of hospital stay and the recovery time around the drainage tube,and reduce the incidence of postoperative complications,with better drainage effect.
