摘要
目的:比较热牙胶垂直加压和连续波热牙胶填充C形根管的临床效果。方法:选择2019年1月-2021年6月在笔者医院就诊的146例下颌第二恒磨牙C形根管充填患者,共146颗患牙,随机分为两组。单纯热牙胶组(73例)患者采用单纯热牙胶垂直加压填充C形根管治疗;连续波热牙胶组(73例)患者采用连续波热牙胶填充C形根管治疗。观察并比较两组C形根管充填时间、充填质量、术后3个月临床效果、疼痛发生率及并发症发生率等。结果:与单纯热牙胶垂直加压相比,连续波热牙胶充填C形根管充填时间短(P<0.001);连续波热牙胶组根管恰填患者比例高于单纯热牙胶组(P<0.05);连续波热牙胶组发生术中疼痛患者比例低于单纯热牙胶组(P<0.01)。两组术后暂时疼痛与术后1周疼痛患者比例差异无统计学意义(P>0.05);连续波热牙胶组总有效率高于单纯热牙胶组(P<0.05),连续波热牙胶组与单纯热牙胶组总并发症发生率比较差异无统计学意义(P>0.05)。两组牙龈红肿、充填物渗漏、细菌感染、根折等并发症的发生率比较差异无统计学意义(P>0.05)。连续波热牙胶组咬合不适发生率低于单纯热牙胶组,差异有统计学意义(P<0.05)。结论:连续波热牙胶垂直充填方法可以快速、高质量地完成根管充填,降低术中疼痛发生率和术后并发症发生率,提高临床疗效。
Objective The purpose of this study was to compare the clinical effects of continuous wave of condensation technique and warm vertical condensation in C-shaped canals filling.Methods A total of 146 patients with C-shaped canals filling of mandibular second permanent molar treated in our hospital from January 2019 to June 2021 were recruited,including 146 affected teeth.Among them,73 patients were treated with C-shaped canals filled by warm vertical condensation.Seventythree patients underwent C-shaped canals filled by continuous wave of condensation technique.Observation and comparison were made between the two groups of C-shaped canals filling time,filling quality,clinical effect,incidence of pain,incidence of complications,etc.Results Compared with warm vertical condensation,the time of continuous wave of condensation technique filling C-shaped canals was less(P<0.001).The proportion of C-shaped canals filling patients with continuous wave of condensation technique was higher than that of C-shaped canals filling patients with warm vertical condensation(P<0.05).The proportion of patients with intraoperative pain in continuous wave of condensation technique group was lower than that in warm vertical condensation(P<0.01).There was no significant difference in the proportion of patients with temporary pain and one week of postoperative pain between warm vertical condensation and continuous wave of condensation technique(P>0.05).There was no significant difference in the proportion of effective patients and significantly effective patients between warm vertical condensation and continuous wave of condensation technique(P>0.05).The total effective rate of continuous wave of condensation technique was higher than warm vertical condensation(P<0.05).There was no significant difference in the incidence of complications between warm vertical condensation and continuous wave of condensation technique(P>0.05).There was no significant difference in the incidence of gingival swelling,filling leakage,bacterial infection,root fracture and other complications between warm vertical condensation and continuous wave of condensation technique(P>0.05).The incidence of occlusal discomfort in continuous wave of condensation technique was lower than that in warm vertical condensation(P<0.05).Conclusion Continuous wave of condensation technique can complete the C-shaped canals filling quickly and with high quality,reduce the incidence of intraoperative pain and postoperative complications,and improve the clinical effect.
