基于辛开苦降法探讨功能性消化不良诊治思路

A Discussion on Diagnosis and Treatment of Functional Dyspepsia Based on Method of Xinkai Kujiang

功能性消化不良(functional dyspepsia,FD)是常见的功能性胃肠疾病,以上腹痛、饱胀、早饱、嗳气等不适为主。具有持续、迁延、反复发作等特点,给患者生活质量带来了严重影响。其发病是多种因素共同作用的结果,除了与内脏高敏感、胃肠动力紊乱、脑肠轴及其作用下的肠道菌群紊乱等胃内环境的改变有关外,还与人们生活方式及饮食结构改变、精神情志等因素相关,各种因素相互作用、相互影响,并非独立作用,故该病在演变过程中并不只是寒性或热性一种属性,临床常见的更多表现为寒热错杂属性,故治疗该病时需注重细化病机,运用辛开苦降之法进行整体调理。本文将基于辛开苦降法探讨FD的诊治思路,旨在为FD的中医治疗提供全新思路。

Functional dyspepsia(FD)is a common functional gastrointestinal disease,with the main symptoms of abdominal pain,fullness,early satiety,belching and other discomfort.It has a serious impact on the quality of patients’life for its characteristics of persistence,delay and repeated attacks.Its pathogenesis is the result of the interaction of various factors.In addition to the changes in the internal gastric environment,such as visceral hypersensitivity,gastrointestinal motility disorder,brain gut axis and its effect on intestinal flora disorder,it is also related to the changes of people’s lifestyle and diet structure,as well as mental and emotional factors.All these factors interact with and on one another.So,in the evolution process of the disease,it is not a cold or a hot attribute,but a hot-cold complex attribute in the clinical practice.Therefore,in the treatment of this disease,attention should be paid to the refinement of the pathogenesis,and take the method of Xinkai Kujiang,which means acrid opening and bitter downbearing,for the overall regulation of the body.This article will discuss the diagnosis and treatment of FD based on the method of Xinkai Kujiang,to provide a new idea for the traditional Chinese medicine treatment of FD.
