超声心动图在慢性肺源性心脏病患者病情评估中的应用价值

摘要 目的探讨超声心动图在慢性肺源性心脏病患者病情评估中的应用价值。方法抽取2019年7月至2021年8月南阳市第一人民医院收治的慢性肺源性心脏病患者92例为研究组,其中根据病情程度分为失代偿者48例及代偿者44例;另抽取同期健康体检者46例为对照组。两组均采取超声心动图检查。比较对照组与研究组及研究组不同病情程度、不同预后患者肺动脉压力(PAP)、三尖瓣舒张早期血流速度/右房室瓣舒张晚期A波峰值的比值(TVE/TVA)、最大肺动脉血流速度、平均肺动脉血流加速度。结果研究组PAP、最大肺动脉血流速度、平均肺动脉血流加速度高于对照组,TVE/TVA低于对照组(P<0.05)。研究组失代偿期者PAP高于代偿期者,TVE/TVA低于代偿期者,最大肺动脉血流速度、平均肺动脉血流加速度大于代偿期者(P<0.05)。研究组死亡者PAP高于存活者,TVE/TVA低于存活者,最大肺动脉血流速度、平均肺动脉血流加速度大于存活者(P<0.05)。结论超声心动图应用在慢性肺源性心脏病患者中,能清晰显示患者右心室功能改变状况与结构形态,有利于临床诊治。 Objective To investigate the value of echocardiography in evaluation of severity of chronic pulmonary heart disease.Methods A total of 92 patients with chronic pulmonary heart disease treated in Nanyang First People’s Hospital from July 2019 to August 2021 were selected as the study group,and they were divided into decompensated patients(48 cases)and compensated patients(48 cases)according to the degree of disease.Another 46 healthy subjects during the same period were selected as the healthy control group.Echocardiography was performed in both groups.Pulmonary artery pressure(PAP),tricuspid valve early-diastolic blood flow velocity/right atrioventricular valve late diastolic of A wave(TVE/TVA),maximum pulmonary artery blood flow velocity and average pulmonary artery blood flow acceleration were compared between the control group and the study group,and were compared between patients with different disease severities and prognoses.Results The PAP,maximum pulmonary artery blood flow velocity and average pulmonary artery blood flow acceleration of the study group were higher than those of the control group,while the TVE/TVA of the study group was lower than that of the control group(P<0.05).The PAP of decompensated patients in the the study group was higher than that of compensated patients,the TVE/TVA of decompensated patients was lower than that of compensated patients,the maximum pulmonary artery blood flow velocity and the average pulmonary artery blood flow acceleration were higher than those of compensated patients(P<0.05).The PAP of the dead was higher than that of the survivors,while TVE/TVA was lower than that of the survivors,moreover,the maximum pulmonary artery blood flow velocity and average pulmonary artery blood flow acceleration were higher than those of the survivors in the study group(P<0.05).Conclusions Echocardiography in the diagnosis of patients with chronic pulmonary heart disease can clearly display the changes of right ventricular function and structure,which is beneficial to clinical diagnosis and treatment.
