胰岛素、维生素D水平在老年性糖尿病患者中的表达及与血糖水平的相关性研究

摘要 目的探讨胰岛素及维生素D水平在老年性糖尿病患者中的表达及与血糖水平的相关性研究。方法选择2019年4月—2022年2月广东省英德市人民医院收治的60例老年性糖尿病患者为观察组;选择糖耐量减低(impaired glucose tolerance,IGT)的老年患者58例为对照1组,选择健康体检老年人59名为对照2组。分别测定各组人员空腹胰岛素(FINS)、空腹血糖(FPG)水平、25-(OH)D、餐后2h血糖(2hPG)及糖化血红蛋白(HbAlc)水平,并依据空腹胰岛素、空腹血糖水平按相关公式计算空腹状态指数胰岛素抵抗指数(HOMAIR)、胰岛β细胞功能(HOMA-β)、胰岛素敏感指数(ISI)。采用Pearson相关性分析软件对老年性糖尿病患者胰岛素、25-(OH)D水平与血糖水平完成相关性分析。结果观察组老年性糖尿病患者FINS、HOMA-IR水平均高于对照1组和对照2组,差异有统计学意义(P<0.05),HOMA-β、ISI及25-(OH)D水平均低于对照1组和对照2组,差异有统计学意义(P<0.05);观察组FPG、2hPG、HbAlc水平高于对照1组和对照2组,差异有统计学意义(P<0.05)。Pearson相关性分析结果表明老年性糖尿病患者FPG、2hPG、HbAlc水平与FINS、HOMAIR水平呈正相关性(P<0.05),与HOMA-β、ISI及25-(OH)D水平呈负相关性(P<0.05)。结论老年性糖尿病患者常伴有维生素D水平下降、胰岛素抵抗增加、胰岛β细胞功能下降,其表达水平与血糖水平存在强相关性。 Objective To investigate the expression of insulin and vitamin D levels in elderly patients with diabetes mellitus and the correlation with blood glucose levels.Methods 60 cases of geriatric diabetic patients admitted to Yingde People’s Hospital of Guangdong Province from April 2019 to February 2022 were selected as the observation group;58 cases of elderly patients with reduced glucose tolerance(IGT)were selected as the control 1 group,59 cases of elderly people with healthy physical examination were selected as the control 2 group.The levels of fasting insulin(FINS),fasting blood glucose(FPG),25-(OH)D,2 h post-meal blood glucose(2 hPG)and glycosylated hemoglobin(HbAlc)in each group were determined,respectively.And fasting insulin and fasting glucose levels were calculated according to the correlation formula of fasting status index insulin resistance index(HOMA-IR),isletβ-cell function(HOMA-β)and insulin sensitivity index(ISI).Pearson correlation analysis software was used to complete the correlation analysis between insulin,25-(OH)D levels and blood glucose levels in elderly diabetic patients.Results The levels of FINS and HOMA-IR in the observation group were higher than those in the control 1 and control 2 groups,the difference was statistically significant(P<0.05);the levels of HOMA-β,ISI and 25-(OH)D were lower than those in the control 1 and control 2 groups,the difference was statistically significant(P<0.05);the levels of FPG,2 hPG and HbAlc in the observation group were higher than those in the control 1 and control 2 groups,the difference was statistically significant(P<0.05).Pearson correlation analysis showed that FPG,2 hPG and HbAlc levels in elderly diabetic patients were positively correlated with FINS and HOMA-IR levels(P<0.05);and negatively correlated with HOMA-β,ISI and 25-(OH)D levels(P<0.05).Conclusion Senile diabetes mellitus is often associated with decreased levels of vitamin D,increased insulin resistance,and decreased function of islet beta cells,whose expression levels are strongly correlated with blood glucose levels.
