妊娠期糖尿病患者孕期体质量的变化情况对患者血糖水平及分娩方式的影响

Influence of Pregnancy Weight Change in Gestational Diabetes on Patients’Blood Glucose Level and Mode of Delivery

目的分析妊娠期糖尿病患者孕期体质量的变化情况对患者血糖水平及分娩方式的影响。方法选取2021年1—12月厦门大学附属妇女儿童医院收治的200例妊娠期糖尿病患者为研究对象,依据患者的孕前体质指数分为对照组(120例,BMI≥24kg/m^(2))和观察组(80例,BMI<24kg/m^(2)),对比两组分娩方式,同时观察组依据孕期体质量增长情况分为观察A组(30例,孕期体质量增长≥15kg)和观察B组(50例,孕期体质量增长<15kg)。对比两组血糖水平及分娩方式。结果观察组剖宫产率为22.5%明显低于对照组的35.83%,差异有统计学意义(χ^(2)=4.026,P=0.045),对照组子痫前期、胎儿窘迫、早产率均明显高于观察组,差异有统计学意义(χ^(2)=6.389,P=0.011)。观察A组平均血糖水平、剖宫产率、子痫前期、胎儿窘迫、早产率明显高于观察B组,差异有统计学意义(P<0.05)。结论妊娠期糖尿病孕妇孕前体质指数越高,孕期血糖水平升高的可能性越大,因此需做好患者的体质量管理。

Objective To analyze the influence of the change of weight during pregnancy in patients with gestational diabetes on the patients’blood glucose level and mode of delivery.Methods From January to December 2021,200 patients with gestational diabetes admitted to the Affiliated Women’s and Children’s Hospital of Xiamen University were selected as the research objects,and were divided into the control group(120 patients,BMI≥24 kg/m~2)and observation group(80 cases,BMI<24 kg/m~2).The delivery methods of the two groups were compared.At the same time,the observation group was divided into observation group A(30 cases,the growth of body mass during pregnancy≥15 kg)and observation group B(50 cases,the growth of body mass during pregnancy<15 kg)according to the growth of body mass during pregnancy.The blood glucose level and delivery mode were compared between the two groups.Results The cesarean section rate in the observation group of 22.5%was significantly lower than that of 35.83%in the control group,the difference was statistically significant(χ^(2)=4.026,P=0.045),the rates of preeclampsia,fetal distress,and premature delivery in the control group were significantly higher than those in the observation group,the difference was statistically significant(χ^(2)=6.389,P=0.011).The average blood glucose level,cesarean section rate,preeclampsia,fetal distress,and premature delivery rate in observation group A were significantly higher than those in observation group B,the difference was statistically significant(P<0.05).Conclusion The higher the pre-pregnancy body mass index of pregnant women with gestational diabetes,the greater the possibility of elevated blood glucose levels during pregnancy,so the weight management of patients needs to be done.
