米非司酮联合米索前列醇对不同孕周终止妊娠的效果比较

Comparison of the effect of mifepristone combined with misoprostol on termination of pregnancy at different gestational weeks

目的 分析米非司酮联合米索前列醇对不同孕周终止妊娠的效果。方法 选取于本院进行终止妊娠干预的100例患者为研究对象,按孕周不同将其分为A组(7周及以内)35例、B组(8~12周)35例及C组(13~18周)30例。所有孕妇均采用米非司酮联合米索前列醇干预,对比三组的终止妊娠成功率及不良反应发生率。结果 在完全流产率方面,A组(94.29%)>B组(77.14%)>C组(50.00%),三组间比较,差异有统计学意义(P<0.05);在不完全流产率及流产失败率方面,A组<B组<C组,三组间比较,差异有统计学意义(P<0.05);在流产成功率方面,A组(100%)>B组(85.71%)>C组(76.67%),三组间比较,差异有统计学意义(P<0.05)。三组间腹痛、恶心呕吐、感染发生率比较,差异有统计学意义(P<0.05);三组间寒战、发热发生率比较,差异无统计学意义(P>0.05)。结论 米非司酮联合米索前列醇能够起到良好的终止妊娠效果,但不同孕周孕妇用药后流产成功率及安全性有较大差异,孕期在7周内的孕妇流产成功率最高、不良反应发生率最低。

Objective To analyze the effect of mifepristone combined with misoprostol on termination of pregnancy at different gestational weeks. Methods A total of 100 patients who received intervention for termination of pregnancy in our hospital were selected as research objects and were divided into 35 cases in group A(7 weeks or less), 35 cases in Group B(8~12 weeks)and 30 cases in group C(13~18 weeks) according to different gestational weeks. All pregnant women were treated with mifepristone combined with misoprostol. The success rates of pregnancy termination and the incidences of adverse reactions among the three groups were compared. Results In the total abortion rate, group A(94.29%) > Group B(77.14%) > group C(50.00%), the differences between the three groups were statistically significant(P<0.05);in terms of incomplete abortion rate and abortion failure rate, group A< group B< group C, the differences between the three groups were statistically significant(P<0.05);In terms of the success rate of abortion, group A(100%) > Group B(85.71%) > group C(76.67), the differences between the three groups were statistically significant(P<0.05). There were statistically significant differences in the incidences of abdominal pain, nausea and vomiting and infection among the three groups(P<0.05);there were no significant differences in the incidences of chills and fever among the three groups(P>0.05). Conclusion Mifepristone combined with misoprostol can play a good role in the termination of pregnancy, but the success rate and safety of abortion in pregnant women with different gestational weeks are significantly different.The success rate of abortion in pregnant women within 7 weeks of pregnancy is the highest and the incidence of adverse reactions is the lowest.
