摘要
目的探讨急性深穿支脑梗死的发病危险因素及对皮层认知功能状态的影响。方法选取医院2021年6月至2022年6月收治的急性深穿支脑梗死患者50例作为研究组,选取同期接受体检的健康者50例作为对照组。收集所有对象病历资料,包括年龄、性别及各相关指标,对两组患者认知功能状态进行比较。以急性脑梗死感染因素作为因变量,采用多变量的Logistic回归对急性深穿支脑梗死的发病危险因素进行分析。结果超敏C反应蛋白(OR=2.145)、高密度脂蛋白胆固醇(OR=3.987)、纤维蛋白原(OR=2.572)是急性深穿支脑梗死的发病因素。研究组患者事件相关单位P300潜伏期显著高于对照组(P<0.05),研究组患者事件相关单位P300波幅和MoCA评分显著低于对照组患者(P<0.05)。结论超敏C反应蛋白、高密度脂蛋白、纤维蛋白原是急性深穿支脑梗死的发病因素,事件相关电位P300与MoCA为早期脑梗死患者认知功能的评估筛查提供了重要客观指标。
Objective To investigate risk factors of acute deep perforator cerebral infarction and their effect on the cerebral cortex cognitive function.Methods A total of 50 patients with acute deep perforator cerebral infarction who received treatment admitted to the hospital from June 2021 to June 2022 were selected as the study group,50 healthy people who received physical examination at the same time were selected as the control group.The medical records of all patients,including age,gender and related indicators,and the cognitive function in the two groups were compared.The risk factors of acute deep perforator cerebral infarction were analyzed by multivariable Logistic regression with infection factors of acute cerebral infarction as dependent variables.Results High sensitivity C-reactive protein(hs-CRP,OR=2.145),high density lipoprotein cholesterol(HDL-C,OR=2.572)and fibrinogen(Fib,OR=3.987)were the pathogenic factors of acute deep perforator cerebral infarction.The P300 latency of event related units in the study group was significantly higher than that in the control group(P<0.05),while the P300 amplitude and Montreal Cognitive Assessment(MoCA)score of the patients in the study group were significantly lower than those in the control group(P<0.05).Conclusion The hs-CRP,LDL-C and Fib are the pathogenic factors of acute deep perforator cerebral infarction.Event related potential P300 and MoCA provide important objective indicators for the evaluation and screening of cognitive function in patients with early cerebral infarction.
