超声对甲状腺癌合并淋巴结转移诊断的准确率分析

Analysis of the accuracy of ultrasonography in the diagnosis of thyroid cancer with lymph node metastasis

目的:在甲状腺癌合并淋巴结转移临床诊断过程中,切实立足于淋巴结超声检查特征以及血流动力学,研究分析超声对此疾病的诊断准确率。方法:随机选取曹县人民医院2022年2月—8月期间所收治的甲状腺癌患者(样本观察数量为520例,视为观察组),结合淋巴结转移状况将其划分为转移组(n=236)与未转移组(n=284),均检测其血流动力学相关指标水平,对转移组实施超声淋巴结穿刺活检。结果:Ⅲ级肿瘤患者血流动力学S与D指标水平均更高,且RI与PI指标水平均更低,差异有统计学意义(P<0.05)。转移组血流动力学RI与PSV指标水平均更低;且淋巴结超声检测异常例数均更多,差异有统计学意义(P<0.05)。结论:在甲状腺癌合并淋巴结转移临床诊断过程中,立足于淋巴结超声检测特征、血流动力学分析,能够提高淋巴结转移的诊断准确率,能够对甲状腺癌临床诊断、淋巴结转移评估提供良好的参考依据。

Objective In the clinical diagnosis of thyroid cancer with lymph node metastasis,based on the ultrasonic characleristics of lymph nodes and hemodynamics,to study and analyze the accuracy of ultrasonic diagnosis of the disease.Methods The patients with thyroid cancer admitted to Caoxian People’s Hospital from February to August 2022 were randomly selected(520 cases were observed,which were regarded as the observation group),and were divided into metastatic group(n=236) and non metastatic group(n=284) according to the status of lymph node metastasis.The levels of hemodynamic related indicators were detected,and ultrasonic lymph node biopsy was performed in the metastatic group.Results The levels of hemodynamics S and D were higher in patients with grade Ⅲ tumor,and RI and PI were lower,the difference is statistically significant(P <0.05).Hemodynamic RI and PSV were lower in the metastatic group;The number of abnormal lymph nodes detected by ultrasound was more,the difference is statistically significant(P <0.05).Conclusion In the clinical diagnosis of thyroid cancer with lymph node metastasis,based on the ultrasonic detection characteristics of lymph nodes and hemodynamic analysis,it can improve the accuracy of the diagnosis of lymph node metastasis,and provide a good reference for the clinical diagnosis and evaluation of lymph node metastasis of thyroid cancer.
