经脐单孔腹腔镜与传统腹腔镜手术治疗卵巢囊肿的效果比较

Comparison of effect of transumbilical single-port laparoscopic surgery versus traditional laparoscopic surgery on ovarian cysts

目的比较经脐单孔腹腔镜与传统腹腔镜手术治疗卵巢囊肿的临床效果。方法抽取2016年11月至2021年11月周口市中心医院收治的卵巢囊肿患者60例,按随机数字表法分为A组与B组,每组30例。B组采用传统腹腔镜手术治疗,A组采用经脐单孔腹腔镜手术治疗。比较两组围术期情况、黄体生成素(LH)、卵泡刺激素(FSH)、视觉模拟评分法(VAS)评分、并发症发生率及满意度。结果A组术中出血量少于B组,住院时间、术后排气时间短于B组(P<0.05);术后,A组LH、FSH与B组比较差异未见统计学意义(P>0.05);术后24、48 h,A组VAS评分低于B组(P<0.05);A组并发症发生率(6.67%,2/30)与B组(16.67%,5/30)比较差异未见统计学意义(P>0.05)。结论与传统腹腔镜手术相比,经脐单孔腹腔镜治疗卵巢囊肿能改善围术期情况,减轻疼痛度,对卵巢功能影响小,安全性良好。

Objective To compare the effect of transumbilical single-port laparoscopic surgery and traditional laparoscopic surgery on ovarian cysts.Methods A total of 60 patients with ovarian cysts treated in Zhoukou Central Hospital from November 2016 to November 2021 were selected,and they were divided into group A and group B according to random number table method,with 30 cases in each group.The group B was treated by traditional laparoscopic surgery,and the group A was treated by transumbilical single-port laparoscopic surgery.The perioperative conditions,luteinizing hormone(LH),follicle-stimulating hormone(FSH),visual analogue scale(VAS)score,complication rate and satisfaction degree were compared between the two groups.Results The intraoperative blood loss in the group A was less than that in the group B,while the hospital stay and postoperative exhaust time were shorter than those in the group B(P<0.05).After operation,there was no significant difference in LH and FSH between the group A and the group B(P>0.05).At 24 and 48 hours after operation,the VAS scores of the group A were lower than those of the group B(P<0.05).There was no significant difference in the incidence of complications between the group A(6.67%,2/30)and the group B(16.67%,5/30),P>0.05.Conclusions Compared with traditional laparoscopic surgery,transumbilical single-port laparoscopy in the treatment of ovarian cyst can improve the perioperative situation,reduce the degree of pain,with less impact on ovarian function and good safety.
