双环醇片联合恩替卡韦治疗慢性乙型肝炎的效果及对肝功能、免疫功能的影响

Efficacy of bicyclol tablets combined with entecavir in the treatment of chronic hepatitis B and its effect on liver function and immune function

目的探讨双环醇片、恩替卡韦治疗慢性乙型肝炎的效果及对肝功能、免疫功能的影响。方法抽取2019年6月至2020年7月安阳市第五人民医院收治的慢性乙型肝炎患者76例,按照随机数字表法分为单一组和联合组,每组38例。单一组采用恩替卡韦治疗,联合组在其基础上采用双环醇片联合治疗。结果联合组总有效率(97.37%,37/38)高于单一组(78.95%,30/38),P<0.05;联合组不良反应发生率(7.89%,3/38)与单一组(2.63%,1/38)比较差异未见统计学意义(P>0.05);联合组CD4^(+)、CD4^(+)/CD8^(+)水平高于单一组(P<0.05);联合组乙肝病毒脱氧核糖核酸(HBV-DNA)、乙型肝炎病毒e抗原(HbeAg)阴转率(97.37%,37/38;94.74%,36/38)高于单一组(78.95%,36/38;68.42%,26/38),P<0.05;治疗后,联合组天门冬氨酸氨基转移酶(AST)、总胆红素(TBIL)水平低于单一组(P<0.05);治疗后,联合组白细胞介素-6、肿瘤坏死因子-α水平低于单一组(P<0.05)。结论双环醇片联合恩替卡韦治疗慢性乙型肝炎能提高治疗效果,改善炎性反应和免疫功能,提高HBV-DNA、HbeAg阴转率,改善肝功能,且具有用药安全性。

Objective To investigate the effect of bicyclol tablets and entecavir on chronic hepatitis B and its influence on liver function and immune function.Methods A total of 76 patients with chronic hepatitis B treated in the Fifth People’s Hospital of Anyang from June 2019 to July 2020 were selected.According to the random number table method,76 patients were divided into combined group and single group,with 38 cases in each group.The single group was treated with entecavir,and the combined group was treated with bicyclol tablets based on the treatment of the control group.Results The total effective rate in the combined group was 97.37%(37/38),which was higher than 78.95%(30/38)in the single group(P<0.05).There was no significant difference in the incidence of adverse reactions between the combined group(7.89%,3/38)and the single group(2.63%,1/38),P>0.05.The levels of cluster of differentiation 4^(+)(CD4^(+))and CD4^(+)/cluster of differentiation 8^(+)(CD8^(+))in the combined group were higher than those in the single group(P<0.05).The negative conversion rates of hepatitis B virus DNA(HBV-DNA)and hepatitis B e-antigen(HbeAg)in the combined group were 97.37%(37/38)and 94.74%(36/38),respectively,which were higher than 78.95%(36/38)and 68.42%(26/38)in single group(P<0.05),correspondingly.After treatment,aspartic transaminase and total bilirubin levels in the combined group were lower than those in the single group(P<0.05).After treatment,the levels of interleukin-6 and tumor necrosis factor-α in the combined group were lower than those in the single group(P<0.05).Conclusions Bicyclol tablets combined with entecavir in the treatment of chronic hepatitis B can improve the therapeutic effect,inflammatory response,immune function,negative conversion rate of HBV-DNA and HbeAg,and liver function,with good drug safety.
