远程慢病药学服务对老年高血压患者血压控制及治疗依从性的影响

Influence of remote pharmaceutical care for chronic disease on blood pressure control and compliance in elderly hypertension

目的探讨临床药师主导的远程慢病学服务对老年高血压患者的影响。方法选取,2020年7月至2021年7月收治的老年高血压患者80例为研究对象。以随机数字表法作为分组标准分为2组,每组40例,对照组(n=40)开展常规慢性病管理;观察组(n=40)开展临床药师主导的远程慢病药学服务。随访6个月后对2组患者的血压控制情况、依从性情况进行比较。结果经不同管理后,2组高血压患者药物知晓率均有所提高,且观察组提升程度明显高于对照组(P<0.05)。出院时2组患者舒张压、收缩压差异无统计学意义(P>0.05);出院6个月显示观察组舒张压、收缩压低于对照组,有统计学意义(P<0.05)。出院6个月显示观察组治疗依从性各项得分均高于对照组,有统计学意义(P<0.05)。观察组服药差错事件发生率为2.50%,明显低于照组的12.50%,差异有统计学意义(P<0.05)。结论对老年高血压患者开展临床药师主导的远程慢病药学服务,能够有效提升血压控制效果以及治疗依从性。

Objective To explore the influence of pharmacist-led remote pharmaceutical care for chronic disease on blood pressure control and compliance in elderly hypertension.Methods Totally 80 elderly hypertension patients admitted to our hospital from July 2020 to July 2021 were as subjects,the patients were randomized to treatment with routine chronic disease management(n=40,control group)or remote pharmaceutical care(n=40,observation group).After follow-up 6 months,the level of blood pressure and compliance were compared.Results After administration,the drug awareness rate of hypertension patients in both groups was improved,and the improvement degree of observation group was significantly higher than that of control group(P<0.05).There was no significant difference in diastolic blood pressure(DBP)and systolic blood pressure(SBP)between groups at discharge(P>0.05).After follow-up 6 months,DBP and SBP in the observation group were significantly lower than those in the control group(P<0.05);meanwhile,the compliance in the observation group were significantly higher than those in the control group(P<0.05).The incidence of medication error events in the observation group was significantly lower than that in the control group(2.50%vs 12.50%,P<0.05,respective).Conclusions pharmacist-led remote pharmaceutical care for chronic diseases would be conducive to the improvement of blood pressure and compliance in elderly hypertension.
