摘要
目的 探讨左室自动功能成像(AFI)对新诊断T2DM患者心脏功能的评估价值。方法 选取2019年6月至2020年12月于我院内分泌科治疗的80例新诊断T2DM患者(T2DM组),同期选取我院35名体检健康者为正常对照(NC)组,运用AFI技术获取左室功能参数,比较各组差异。二元Logistic回归分析左室整体长轴应变均值(GLPS-AVG)的影响因素。结果 与NC组比较,T2DM组左室舒张末期内径、舒张末期室间隔厚度(IVSD)、舒张早期与末期峰值流速比值(E/A)升高,左室三腔心收缩峰值应变、四腔心收缩峰值应变、二腔心收缩峰值应变、GLPS-AVG降低(P<0.05)。二元Logistic回归分析显示,BMI、IVSD、左室舒张末期后壁厚度、E/A是GLPS-AVG降低的危险因素。结论 左室AFI可早期发现T2DM患者左室收缩功能改变。BMI是新诊断T2DM患者GLPS-AVG降低的影响因素。
Objective To apply ultrasonic left ventricular automatic functional imaging(AFI)technology to assess left ventricular function in newly diagnosed type 2 diabetes mellitus(T2DM).Methods 80 newly diagnosed T2DM patients(T2DM group)treated in our hospital from June 2019 to December2020 were selected. 35 healthy people were selected as the normal control(NC)group at the same time.The left ventricular function parameters were obtained by AFI technology. Binary logistic regression analysis was used to evaluate the influencing factors of left ventricular global long axis strain mean(GLPS-AVG).Results Compared with the NC group,the left ventricular end diastolic inner diameter,end diastolic ventricular septal thickness(IVSD),the ratio of early diastolic to end diastolic peak flow velocity(E/A)increased in T2DM group,and the left ventricular three chamber systolic peak strain,four chamber systolic peak strain,two chamber systolic peak strain,and GLPS-AVG decreased(P<0. 05). Binary logistic regression analysis showed that BMI,IVSD,left ventricular end diastolic posterior wall thickness and E/A were the risk factors for GLPS-AVG reduction.Conclusion AFI can early detect the changes of left ventricular systolic function in T2DM patients. BMI is an important influencing factor on the decrease of GLPS-AVG in newly diagnosed T2DM patients.
