生殖系统肿瘤放疗患者癌因性疲乏与心境状态、应对方式的关系研究

Study on the relationship between cancer-caused fatigue and state of mind,coping style and countermeasures in patients undergoing radiotherapy for genital tumors

目的:探讨生殖系统肿瘤放疗患者癌因性疲乏与心境状态、应对方式的关系。方法:选取2019年10月至2021年10月我院120例生殖系统肿瘤放疗患者作为研究对象,采用癌症疲乏量表(CFS)、简明心境问卷(POMS-SF)、简易应对问卷(SCSQ)评价癌因性疲乏、心境状态、应对方式,并对各变量间进行相关分析、回归分析及中介作用分析。结果:生殖系统肿瘤放疗患者CFS总分为(32.56±6.67)分,SCSQ-积极应对总分为(16.66±2.68)分,消极应对总分为(21.75±1.06)分,POMS-SF总分为(134.76±10.03)分;Pearson相关性分析显示,癌因性疲乏各维度及总分与消极应对、心境状态呈正相关(P<0.05);多元线性回归分析显示,消极应对、心境状态对生殖系统肿瘤放疗患者癌因性疲乏具有预测作用(P<0.05);多重中介效应模型和Bootatrap检验结果显示,心境状态可通过消极应对方式间接影响癌因性疲乏(χ^(2)/df=1.833,P=0.074,GFI=0.983,AGFI=0.955,NFI=0.981,RFI=0.950,IFI=0.992,TLI=0.982,CFI=0.988,RMSEA=0.056),中介效应效果量为35.71%。结论:生殖系统肿瘤放疗患者癌因性疲乏状态处于中等水平,临床实际中可通过音乐疗法、意象放松训练等改善其心境状态,强化其治疗信心,采取积极应对方式,以此减轻癌因性疲乏,促进身心健康。

Objective:To investigate the relationship between cancer-caused fatigue and state of mind,coping style and countermeasures in patients undergoing radiotherapy for genital tumors.Methods:120 patients undergoing radiotherapy for genital tumors in our hospital from October 2019 to October 2021 were selected as study subjects,and the Cancer Fatigue Scale(CFS),Brief Mood State Questionnaire(POMS-SF),and Simple Coping Questionnaire(SCSQ)were used to evaluate cancer-caused fatigue,mood state,and coping style.Correlation analysis,regression analysis,and mediating effect analysis were performed among the variables.Results:Total CFS score was 32.56±6.67 points,total SCSQ-positive coping score was(16.66±2.68)points,total negative coping score was(21.75±1.06)points,and total POMS-SF score was(134.76±10.03)points in patients undergoing radiotherapy for genital tumors.Pearson correlation showed that the dimensions and total scores of cancer-caused fatigue were positively correlated with state of mind and negative coping.Multiple linear regression analysis showed that negative coping and state of mind had predictive effect on cancer-caused fatigue in patients undergoing radiotherapy for genital tumors(P<0.05).Multiple mediated effects model and Bootatrap test showed that state of mind could indirectly influence cancer-caused fatigue through negative coping(χ^(2)=1.833,P=0.074,GFI=0.983,AGFI=0.955,NFI=0.981,RFI=0.950,IFI=0.992,TLI=0.982,CFI=0.988,RMSEA=0.056),with a mediated effect size of 35.71%.Conclusion:In clinical practice,music therapy and imagery relaxation training can be used to improve the state of mind in patients undergoing radiotherapy for genital tumors,strengthen their confidence in treatment,and adopt positive coping methods to reduce cancer-caused fatigue and promote physical and mental health.
