心电图引导下新生儿脐静脉置管法在早产儿脐静脉置管中的临床应用效果

Clinical application effect of the electrocardiogram guided neonatal umbilical vein catheterization in premature neonates

目的探讨心电图(ECG)引导下脐静脉置管(UVC)法在早产儿UVC中的临床应用效果。方法选取2020年3月至2022年7月江西省妇幼保健院收治的200例早产儿作为研究对象,采用随机数字表法分为实验组和对照组,每组各100例。实验组采用心电图引导下新生儿UVC法,对照组采用常规置管方法。比较两组UVC情况及并发症的发生率。结果实验组UVC 1次置管成功率及总成功率均高于对照组,置管时间长于对照组,置管前和置管时的血氧浓度均高于对照组,在UVC操作时及留置期间的并发症总发生率均低于对照组,差异有统计学意义(P<0.05)。结论心电图引导下新生儿UVC法在早产儿UVC中,能有效地提高脐静脉导管置管的成功率,有利于维持患儿生命体征稳定,降低置管过程中的并发症、延长患儿置管后的留置时间。

Objective To investigate the clinical application effect of electrocardiogram(ECG)guided umbilical vein catheterization(UVC)in premature neonates.Methods A total of 200 premature neonates admitted to Jiangxi Maternal and Child Health Hospital from March 2020 to July 2022 were selected as study subjects.The cases were divided into experimental group and control group by random number table method,100 cases in each group.The experimental group used ECG guided neonatal UVC,while the control group used conventional catheterization.The clinical application effect of UVC and complications were compared between the two groups.Results The success rate of one try of catheterization and total success rate of UVC in the experimental group were higher than those in the control group;the retention time of catheter was longer than that of control group;the blood oxygen concentration before and during catheterization was higher than that of the control group,the total incidence of complications during the operation of UVC and during the retention period was lower than that of the control group.The differences were statistically significant(P<0.05).Conclusion ECG guided neonatal UVC can effectively improve the success rate of UVC in premature neonate,help to maintain the stability of vital life signs,reduce the incidence of complications in the process of catheterization,and prolong the retention time after catheterization.
