无创心输出量监测在重度子痫前期患者术后的应用

Application of non-invasive cardiac output measurement in postoperative patients with severe preeclampsia

目的探讨无创心输出量监测(NICOM)在重症监护室(ICU)重度子痫前期患者术后的应用价值。方法回顾性分析2014年9月至2021年6月苏州市第九人民医院重症医学科收治的31例重度子痫前期术后患者的临床资料。根据是否行NICOM分为两组,对照组(n=14)采用传统保守治疗,实验组(n=17)在对照组的基础上采用NICOM。比较两组的术后24 h补液量、机械通气时间、肺水肿发生率及住ICU时间。结果实验组术后24 h的补液量低于对照组,机械通气时间、住ICU时间短于对照组(P<0.05)。两组肺水肿发生率比较,差异无统计学意义(P>0.05)。结论NICOM应用于重度子痫前期术后患者中,能降低24 h补液量、缩短机械通气时间及住院时间,安全有效且操作简单。

Objective To explore the application value of non-invasive cardiac output measurement(NICOM)in postoperative patients with severe preeclampsia in intensive care unit(ICU).Methods The clinical data of 31 patients with severe preeclampsia admitted to the Department of Critical Care Medicine,Suzhou Ninth People’s Hospital from September 2014 to June 2021 were retrospectively analyzed.The patients were divided into two groups according to whether they underwent NICOM or not.The control group(n=14)was treated with traditional conservative treatment,and the experimental group(n=17)was given NICOM on the basis of the control group.The amount of postoperative fluid infusion in 24 h,the time of mechanical ventilation,the incidence of pulmonary edema and the time of ICU stay were compared between the two groups.Results The amount of postoperative fluid infusion in 24 h in the experimental group was lower than that in the control group,and the time of mechanical ventilation and ICU stay was shorter than that in the control group(P<0.05).There was no statistically significant difference in the incidence of pulmonary edema between the two groups(P>0.05).Conclusion The application of NICOM in postoperative patients with severe preeclampsia can reduce the amount of postoperative fluid infusion in 24 h,shorten the time of mechanical ventilation and the length of hospital stay,which is safe,effective and simple.
