维持性血液透析患者自体动静脉内瘘失功的影响因素分析

Analysis of related factors affecting the failure of autologous arteriovenous fistula in patients with maintenance hemodialysis

目的:分析维持性血液透析(MHD)患者自体动静脉内瘘(AVF)失功的相关影响因素。方法:回顾性分析2018年1月—2021年12月收治的MHD患者78例,根据是否发生AVF失功分为AVF失功组和AVF通畅组,每组39例,查阅并记录患者相关资料,纳入可能的影响因素,经Logistics回归分析MHD患者AVF失功的相关影响因素。结果:AVF失功组患者合并糖尿病、营养不良、高磷血症例数及血清脂蛋白相关磷脂酶A2(Lp-PLA2)水平均高于AVF通畅组(P<0.05)。Logistic回归分析显示,合并糖尿病、营养不良、高磷血症及血清Lp-PLA2过表达均为MHD患者AVF失功的影响因素(OR>1,P<0.05)。结论:MHD患者AVF失功可能受合并糖尿病、营养不良、高磷血症及血清Lp-PLA2过表达等因素的影响。

Objective:To analyze the related factors of autogenous arteriovenous internal fistula(AVF)loss in patients with maintenance hemodialysis(MHD).Methods:A retrospective analysis of 78 patients with MHD admitted between January 2018 and December 2021.The patients were divided into AVF failure group and AVF patency group according to whether AVF failure occurred or not,39 cases in each group.The relevant data of patients were consulted and recorded,and the possible influencing factors were included.The relevant influencing factors of AVF loss in MHD patients were analyzed by Logistics regression.Results:The proportions of diabetes mellitus,malnutrition,hyperphos-phatemia and serum LP-PLA2 level in AVF dysfunction group were higher than those in AVF dysfunction group(P<0.05).Logistic regression analysis showed that diabetes mellitus,malnutrition,hyperphosphatemia and serum LP-PLA2 overexpression were the influencing factors of AVF loss in MHD patients(OR>1,P<0.05).Conclusion:The loss of AVF function in MHD patients may be affected by diabetes mellitus,malnutrition,hyperphosphatemia and serum LP-PLA2 overexpression.
