快速康复理念下骨科全麻术后体位护理改进的效果评价

Orthopaedic General Anesthesia Under the Concept of Rapid Rehabilitation

探讨快速康复理念下骨科全麻术后体位护理改进的效果比较。方法:回顾性分析天津市北辰医院2017年2月-2021年12月收治的478例骨科全麻术后患者,根据护理模式分为观察组和对照组,各239例。对照组术后常规去枕平卧6-8小时,头偏向一侧,而观察组给予快速康复理念下骨科全麻术后体位护理改进,包括术后予垫枕卧位,生命体征平稳后自由卧位。观察2组患者不良反应和疼痛评分有无差异。结果:观察组呕吐、头痛头晕、颈肩腰背紧张、下肢麻木不适并发症显著低于对照组,且有统计学差异(P<0.05);观察组不同时间点VAS疼痛评分显著低于对照组(P<0.05);观察组患者中剧烈疼痛患者比例显著低于对照组,且有统计学差异(P<0.05)。结论:快速康复理念下骨科全麻术后体位护理改进可以显著降低患者不良反应发生风险,降低疼痛评分。

To explore the effect of improving posture nursing after orthopaedic general anesthesia un-der the concept of rapid rehabilitation. Methods:478 orthopaedic patients after general anesthesia in Tianjin Beichen hospital were retrospectively analyzed. According to the nursing mode, they were divided into observation group and control group with 239 cases each. In the control group, the head was tilted to one side for 6-8 hours after opera-tion, while in the observation group, the body position nursing improvement after orthopaedic general anesthesia un-der the concept of rapid rehabilitation included pillow supine position after operation and free supine position after sta-ble vital signs. Observe the difference of adverse reactions and pain scores between the two groups. Results: The complications of vomiting, headache, dizziness, tension of neck, shoulder and back, numbness and discomfort of lower limbs in the observation group were significantly lower than those in the control group ( P < 0 . 05 );The VAS pain score of the observation group at different time points was significantly lower than that of the control group ( P <0 . 05 );The proportion of patients with severe pain in the observation group was significantly higher than that in the control group ( P < 0 . 05 ) . Conclusion: The improvement of posture nursing after orthopaedic general anesthesia under the concept of rapid rehabilitation can significantly reduce the risk of adverse reactions and reduce pain scores.
