摘要
通过对《中华医典》中有关玻璃体混浊的古代文献进行整理归纳,探讨其病名、病因病机、治则及预后。古代医家将本病归为“云雾移睛”“黑花飞蝇”“蝇翅黑花”“眼风黑花”等范畴,属内障眼病。其病多为虚证,病因多为情志内伤、房劳饮食不节,加之以感受风寒邪毒,久病耗伤,致脏腑虚弱,邪从风走而精散,肝肾受伤而津液枯竭,脏腑不能升运精华化源,风痰火邪内生而发为此病。古籍中治疗以补肾益精、清肝滋阴、补益肝肾、祛风清热明目为主,外治以点眼、熏药、敷药治疗为主。不同时期的医家对治疗本病有着各自的辨治方法,现代治疗本病当需结合古代医家思想,为治疗本病增添更坚实的理论依据,用于指导现代中医临床。
Through sorting out the ancient documents about vitreous opacity in the Chinese Medical Dictionary,the paper discusses its name,etiology,pathogenesis,treatment,and prognosis.Ancient physicians classified the disease as Yunwuyijing,Heihuafeiying,Yingchiheihua,Yanfengheihua and other categories.It belongs to cataract eye disease,and its syndromes are mostly related to deficiency,its causes are internal injuries due to moodiness,improper diet and sexual overindulgence,coupled with pathogenic factors of cold and wind,long-term illness and consumption,weakened viscera and bowels,the evils go with the wind and the essence is scattered,the liver and kidney are injured leading to fluid and humor exhaustion,and the viscera and bowels failed to upbear and transport the essence of the source.Wind,phlegm,and fire pathogens are internally generated for this disease.In the ancient books,the treatment is mainly based on tonifying kidney and replenishing essence,clearing liver and nourishing Yin,replenishing liver and kidney,dispelling wind,clearing heat and brightening eyesight.For external treatment,eye drops,steaming preparation and external application agents are major modalities.Physicians in different periods have their own ways of syndrome differentiation and treatment for this disease.Modern treatment of this disease should be combined with ancient medical thoughts to add a more solid theoretical basis for the treatment of this disease and to guide the clinical practice of modern Chinese medicine.
