多层螺旋CT评价心外膜脂肪厚度与冠状动脉疾病相关性研究 被引量:1

摘要 目的:本研究采用多层螺旋CT评价心外膜脂肪厚度(epicardial fat thickness,EFT)与冠状动脉疾病(coronary artery disease,CAD)之间的相关性。方法:80例2019年1月至2020年8月疑似冠心病患者,在我院进行64排(SIEMENS SOMATOM Perspective)多层螺旋CT进行冠状动脉扫描并同期(一周内)进行冠状动脉造影(coronary angiography,CAG),观察冠脉病变的程度,并测量EFT。按照冠脉狭窄的程度分为冠心病组(狭窄程度≥50%)和非冠心病组(正常或狭窄程度<50%),比较两组之间EFT值的差异,并评估EFT对冠心病的预测价值。结果:CAD组的平均EFT明显高于对照组(5.15±1.33 mm vs.4.28±1.09 mm,P<0.001)。EFT预测冠心病的最佳截点为≥4.70mm,敏感性为97%,特异度为62%,曲线下面积(area under curve,ACU)为0.792(P<0.001)。多因素回归分析显示,EFT是CAD的独立预测因子。结论:冠心病患者较对照组有EFT值更高,EFT对冠心病有较好的预测价值。
