急诊科过渡时间就诊对急性缺血性卒中溶栓治疗的影响

Effect of Emergency Department Transition Time Visit on Thrombolytic Therapy for Acute Ischemic Stroke

目的探讨并分析急诊科过渡时间就诊对急性缺血性卒中溶栓治疗的影响。方法将我院2021年4月至2022年3月期间收治的104例急性缺血性卒中患者按照就诊时间的不同分成对照组和实验组,每组52例。在非换班期间就诊的52例患者为对照组,而在换班期间进入急诊科就诊的52例患者则为实验组,比较两组患者的溶栓治疗结局。结果对照组的静脉开通时间、CT完成时间、DNT和脱离危险时间均明显短于实验组(P<0.05);对照组的致残率和死亡率与溶栓情况与实验组的差异并不显著(P>0.05);对照组的治疗满意度为98.07%,其显著高于实验组的80.76%(P<0.05);实验组的神经功能缺损评分明显高于对照组(P<0.05);对照组的日常生活能力评分和生存质量明显高于实验组(P<0.05)。结论急诊科过渡时间就诊会极大地影响急性缺血性卒中溶栓治疗的效果,一方面会缩短患者的救治时间,增加神经功能受损程度,另一方面还会影响其预后的独立能力以及生存状态,进而就会降低患者对治疗的满意度,无法确保其康复效果。

Objective To explore and analyze the effect of emergency department transition time visits on thrombolytic therapy for acute ischemic stroke.Methods A total of 104 patients with acute ischemic stroke admitted to our hospital from April 2021 to March 2022 were divided into a control group and an experimental group according to the time of visit,with 52 cases in each group.The 52 patients who visited the clinic during the nonshift period were the control group,and the 52 patients who visited the emergency department during the shift period were the experimental group.The outcomes of thrombolytic therapy were compared between the two groups.Results The vein patency time,CT completion time,DNT and risk-free time in the control group were significantly shorter than those in the experimental group(P<0.05).(P>0.05);the treatment satisfaction of the control group was 98.07%,which was significantly higher than 80.76%of the experimental group(P<0.05);the neurological deficit score of the experimental group was significantly higher than that of the control group(P<0.05);the control group The daily living ability score and quality of life of the group were significantly higher than those of the experimental group(P<0.05).Conclusion The transition time of emergency department visits will greatly affect the effect of thrombolytic therapy for acute ischemic stroke.On the one hand,it will shorten the patient’s treatment time and increase the degree of neurological impairment.On the other hand,it will also affect their independent ability and prognosis of prognosis.Survival status,which in turn will reduce the patient’s satisfaction with treatment and cannot ensure its recovery effect.
