ADC、eADC和rADC值对TIC Ⅱ型、BI-RADS4类乳腺肿瘤良恶性鉴别诊断价值

摘要 目的 探讨扩散加权成像定量参数表观扩散系数、指数表观扩散系数和相对表观扩散系数在TIC Ⅱ型、BI-RADS 4类乳腺肿瘤良恶性鉴别诊断中的价值。方法 回顾性分析我院经磁共振诊断为TICⅡ型、BI-RADS 4类的乳腺肿瘤74例,分别计算病灶的ADC、eADC和r ADC值。采用ROC曲线分析各参数对良恶性病变的鉴别诊断效能。结果 ADC、eADC和r ADC值在乳腺良恶性病变组间的差异均有统计学意义(P<0.05)。ADC、eADC、rADC1和rADC2值鉴别诊断良恶性病变AUC分别为0.916、0.915、0.759、0.769;以1.22、0.354、0.636和0.617作为ADC、eADC、r ADC1和r ADC2最佳诊断阈值,其诊断的敏感度分别为88.4%、97.7%、69.8%、51.2%,特异度为87.1%、74.2%、74.2%、93.5%。结论 ADC、e ADC和r ADC的定量分析是鉴别诊断TICⅡ型、BI-RADS 4类乳腺良恶性肿瘤的有效手段。 Objective To investigate the diagnosis value of the apparent diffusion coefficient(ADC)、exponential apparent diffusion coefficient(e ADC) and the relative apparent diffusion coefficient(r ADC) in the differentiation between benign and malignant breast tumors as TIC type Ⅱ、BI-RADS 4 in diffusion weighted imaging(DWI). Methods A retrospective analysis of 74 cases of breast tumors diagnosed of TIC type Ⅱ and BI-RADS 4 by MRI in our hospital was retrospectively analyzed,and the ADC,e ADC and r ADC values of the lesions were calculated. The receiver operating characteristic curve(ROC) was used to analyze the differential diagnosis power of each parameter for benign and malignant lesions. Results ADC,e ADC and r ADC values were significantly different between the benign and malignant breast lesion groups(P<0.05). ADC,e ADC,r ADC1 and r ADC2 value differential diagnosis AUC of benign and malignant lesions are 0.916,0.915,0.759,0.769;With 1.22,0.354,0.636,and 0.617 as the best diagnostic thresholds for ADC,e ADC,r ADC1 and r ADC2,the sensitivity of the diagnosis is 88.4%,97.7%,69.8%,51.2%,and the specificity is 87.1%,74.2%,74.2 %,93.5%. Conclusion The quantitative analysis of ADC,e ADC and r ADC is an effective method for differential diagnosis of the benign and malignant breast tumors of TIC type Ⅱand BI-RADS4.
