摘要
目的应用3.0T磁共振扩散加权成像(DWI)、磁共振动态增强(DCE-MRI)分析前列腺良恶性病变的鉴别诊断价值。方法将我院2018年7月至2020年9月接收的86例前列腺病变患者,均进行常规MRI、DWI、DCE-MRI检查,本研究以病理结果作为“金标准”,分析常规MRI、DWI、DCE-MRI检查在前列腺病变中的检查结果。病理诊断结果显示:前列腺(癌)恶性病变有37例,前列腺(增生、前列腺炎)良性病变39例、10例。结果DWI检查准确度(96.51%)、敏感度(94.59%)、特异度(97.96%),高于常规MRI准确度(74.42%)、敏感度(70.27%)、特异度(77.55%)(P<0.05);恶性病变组在b值为600s/mm^(2)、800s/mm^(2)、1000s/mm^(2)时,ADC值低于良性病变组(P<0.05);两组T_(max)、SI_(max)、Rmax对比差异显著(P<0.05),良性病变组Tmax高于恶性病变组,而SI_(max)、Rmax均低于恶性病变组。结论3.0T磁共振DWI、DCE-MRI在前列腺良恶性病变鉴别诊断中具有较高的价值。
Objective To analyze the value of 3.0T Mr diffusion-weighted imaging(DWI)and dynamic contrastenhanced Mr imaging(DCE-MRI)in the differential diagnosis of benign and malignant prostate lesions.Methods From July 2018 to September 2020,86 patients with prostate diseases were examined by routine MRI,DWI and DCE-MRI.In this study,the pathological results were taken as the gold standard,and the results of routine MRI,DWI and DCE-MRI in prostate lesions were analyzed.The results of pathological diagnosis showed that there were 37 cases of malignant lesions of prostate(carcinoma)and 39 cases and 10 cases of benign lesions of prostate(hyperplasia and prostatitis).Results The accuracy(96.51%),sensitivity(94.59%)and specificity(97.96%)of DWI were higher than those of conventional MRI(74.42%),sensitivity(70.27%)and specificity(77.55%)(P<0.05).When b values were 600s/mm^(2),800s/mm^(2)and 1000s/mm^(2),the ADC value of malignant disease group was lower than that of benign disease group(P<0.05).There were significant differences in Tmax,SImaxand Rmaxbetween the two groups.Tmaxin the benign lesion group was higher than that in the malignant lesion group,while SImaxand Rmaxwere lower than those in the malignant lesion group.Conclusion 3.0T magnetic resonance DWI and DCE-MRI are of high value in the differential diagnosis of benign and malignant prostate lesions.
