Degree of Weight Loss After Laparoscopic Sleeve Gastrectomy in Patients With Simple Obesity of Different Body Mass Index

目的 探讨不同体重指数(body mass index,BMI)的单纯性肥胖患者腹腔镜胃袖状切除术(laparoscopic sleeve gastrectomy, LSG)后体重下降程度。方法 回顾性分析2019年10月~2022年2月我科符合纳入标准的92例LSG资料,其中Ⅰ级肥胖(30≤BMI≤34.9)45例,Ⅱ级肥胖(35≤BMI≤39.9)30例,Ⅲ级肥胖(BMI≥40)17例。比较3组术后6个月多余体重减轻百分比(percentage of excess weight loss,%EWL)。采用Spearman相关分析计算术前BMI与术后1、3、6个月%EWL的相关性。结果 术后1、3、6个月Ⅰ级肥胖%EWL均明显高于Ⅱ级、Ⅲ级肥胖(P<0.05),Ⅱ级肥胖与Ⅲ级肥胖差异无统计学意义(P>0.05)。术前BMI与术后1、3、6个月%EWL存在负相关关系(r=-0.265,P=0.011;r=-0.347,P<0.001;r=-0.422,P<0.001)。结论 术前BMI越小,LSG术后体重下降程度越大。

Objective To investigate the extent of weight loss after laparoscopic sleeve gastrectomy(LSG) in simple obese patients with different body mass index(BMI). Methods Clinical data of 92 LSG cases that met the inclusion criteria in our department between October 2019 and February 2022 were retrospectively analyzed, including 45 cases of class Ⅰ obesity(30≤BMI≤34.9), 30 cases of class Ⅱ obesity(35≤BMI≤39.9), and 17 cases of class Ⅲ obesity(BMI≥40). The percentage of excess weight loss(%EWL) at 6 months after surgery was compared among the three groups. The Spearman correlation analysis was used to calculate the correlation between preoperative BMI and %EWL at 1, 3 and 6 months after surgery. Results The %EWL of class Ⅰ obesity was significantly higher than class Ⅱ and class Ⅲ obesity at 1, 3 and 6 months after surgery(P<0.05), and the difference between class Ⅱ and class Ⅲ obesity was not statistically significant(P>0.05). There was a negative correlation between preoperative BMI and %EWL at 1, 3, and 6 months postoperatively(r=-0.265, P=0.011;r=-0.347, P<0.001;r=-0.422, P<0.001). Conclusion The smaller the preoperative BMI, the greater the degree of postoperative weight loss after LSG.
