补肾清热汤对肾虚湿热型强直性脊柱炎患者的临床疗效

Clinical effects of Bushen Qingre Decoction on patients with ankylosing spondylitis due to Kidney Deficiency with Dampness-heat

目的 探讨补肾清热汤对肾虚湿热型强直性脊柱炎患者的临床疗效。方法 108例患者随机分为对照组和观察组,每组54例,对照组给予甲氨蝶呤,观察组在对照组基础上加用补肾清热汤,疗程8周。检测临床疗效、IL-18、IL-37、BMP-2、ESR、COX-2、血清免疫球蛋白(IgG、IgM)、BASDAI评分、中医证候评分、SPARCC评分、不良反应发生率变化。结果 观察组总有效率高于对照组(P<0.05),不良反应发生率更低(P<0.05)。治疗后,2组IL-18、IL-37、COX-2、ESR、BASDAI评分、中医证候评分、SPARCC评分降低(P<0.05),BMP-2、血清免疫球蛋白升高(P<0.05),以观察组更明显(P<0.05)。结论 补肾清热汤可安全有效地减轻肾虚湿热型强直性脊柱炎患者炎症反应,抑制疼痛介质分泌,调节免疫功能,促进腰椎功能改善,从而提高临床疗效。

AIM To explore the clinical effects of Bushen Qingre Decoction on patients with ankylosing spondylitis due to Kidney Deficiency with Dampness-heat.METHODS One hundred and eight patients were randomly assigned into control group(54 cases) for 8-week administration of methotrexate, and observation group(54 cases) for 8-week administration of both Bushen Qingre Decoction and methotrexate. The changes in clinical efficacy, IL-18, IL-37, BMP-2, ESR, COX-2, serum immunoglobulins(IgG, IgM), BASDAI score, TCM syndrome scores, SPARCC score and incidence of adverse reactions were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05), along with lower incidence of adverse reactions(P<0.05). After the treatment, the two groups displayed decreased IL-18, IL-37, COX-2, ESR, BASDAI score, TCM syndrome scores and SPARCC score(P<0.05), and increased BMP-2 and serum immunoglobulins(P<0.05), especially for the observation group(P<0.05).CONCLUSION For the patients with ankylosing spondylitis due to Kidney Deficiency with Dampness-heat, Bushen Qingre Decoction can safely and effectively reduce inflammatory reactions, inhibit the secretion of pain mediators, regulate immune functions, and promote the improvement of lumbar spine functions, thus enhance clinical efficacy.
