云南白药联合艾司奥美拉唑治疗十二指肠溃疡出血的效果及对输血量的影响

Effect of Yunnan Baiyao combined with esomeprazole in patients with bleeding of duodenal ulcer and its influence on blood transfusion volume

目的探讨云南白药联合艾司奥美拉唑治疗十二指肠溃疡出血的效果。方法选取2019年3月至2020年6月收治的80例十二指肠溃疡出血患者作为研究对象,按随机数字表法将其分为对照组和观察组,每组40例。对照组采用艾司奥美拉唑治疗,观察组采用云南白药联合艾司奥美拉唑治疗。比较两组的治疗效果。结果治疗后,观察组的中医证候积分、超敏C反应蛋白(hs-CRP)、皮质醇(COR)水平均低于对照组(P<0.05)。观察组的24 h止血率、治疗总有效率均高于对照组,再出血率低于对照组(P<0.05)。观察组的出血量、输血量均少于对照组,止血时间、住院时间均短于对照组,不良反应总发生率低于对照组(P<0.05)。结论云南白药联合艾司奥美拉唑治疗十二指肠溃疡出血的效果显著,可以改善患者的胃脘灼痛、口干等症状,下调血清hs-CRP、COR水平,较好地控制输血量与出血复发情况,且安全性较高。

Objective To investigate the effect of Yunnan Baiyao combined with esomeprazole in patients with bleeding of duodenal ulcer.Methods A total of 80 patients with bleeding of duodenal ulcer admitted from March 2019 to June 2020were selected as the research objects and divided into control group and observation group according to random number table method,with 40 cases in each group.The control group was treated with esomeprazole,and the observation group was treated with Yunnan Baiyao combined with esomeprazole.The therapeutic effects of the two groups were compared.Results After treatment,the TCM syndrome score,hypersensitive C-reactive protein(hs-CRP)and cortisol(COR)levels in the observation group were lower than those in the control group(P<0.05).The 24 h hemostatic rate and total effective rate of treatment in the observation group were higher than those in the control group,and the rebleeding rate was lower than that in the control group(P<0.05).The blood loss volume and blood transfusion volume in the observation group were less than those in the control group,the time of hemostasis and hospital stay were shorter than those in the control group,and the total incidence of adverse reactions was lower than that in the control group(P<0.05).Conclusion Yunnan Baiyao combined with esomeprazole has a significant effect on the treatment of bleeding of duodenal ulcer.It can improve the symptoms of patients such as gastric burning and dry mouth,down-regulate the levels of serum hs-CRP and COR,better control the blood transfusion volume and bleeding recurrence,and has high safety.
