右美托咪定联合罗哌卡因腹横平面阻滞对小儿腹腔镜隐睾固定术麻醉效果及苏醒质量的影响

Effects of dexmedetomidine combined with ropivacaine transversus abdominis plane block on anesthesia effect and recovery quality of laparoscopic cryptorchidism fixation in children

目的探讨右美托咪定联合罗哌卡因腹横平面阻滞对小儿腹腔镜隐睾固定术麻醉效果及苏醒质量的影响。方法选取2019年6月至2021年6月我院收治的100例腹腔镜隐睾固定术患儿作为研究对象,以随机数字表法将其分为对照组和观察组,各50例。对照组采用罗哌卡因腹横平面阻滞,观察组采用右美托咪定联合罗哌卡因腹横平面阻滞。比较两组的麻醉效果及苏醒质量。结果阻滞结束10 min(T1)、切皮时(T2)、术毕(T3)时,观察组的心率(HR)、平均动脉压(MAP)均低于对照组(P<0.05)。T1、T2、T3时,观察组的皮质醇(Cor)及肾上腺素(E)水平均低于对照组(P<0.05)。观察组的拔管时间、清醒时间、离室时间均短于对照组,躁动评分低于对照组(P<0.05)。结论右美托咪定联合罗哌卡因腹横平面阻滞用于小儿腹腔镜隐睾固定术中有助于提高麻醉效果和苏醒质量,值得推广。

Objective To explore the effects of dexmedetomidine combined with ropivacaine transversus abdominis plane block on anesthesia effect and recovery quality of laparoscopic cryptorchidism fixation in children.Methods A total of 100children with laparoscopic cryptorchidism fixation admitted in our hospital from June 2019 to June 2021 were selected as the research objects and divided into control group and observation group by random number table method,with 50 cases in each group.The control group was treated with ropivacaine transversus abdominis plane block,and the observation group was treated with dexmedetomidine combined with ropivacaine transversus abdominis plane block.The anesthetic effect and recovery quality of the two groups were compared.Results At 10 min after block(T1),skin incision(T2),end of operation(T3),the heart rate(HR)and mean arterial pressure(MAP)in the observation group were lower than those in the control group(P<0.05).At T1,T2,T3,the levels of cortisol(Cor)and epinephrine(E)in the observation group were lower than those in the control group(P<0.05).The extubation time,awake time,leaving room time in the observation group were shorter than those in the control group,and the agitation score was lower than that in the control group(P<0.05).Conclusion Dexmedetomidine combined with ropivacaine for transversus abdominis plane block in laparoscopic cryptorchidism fixation in children is helpful to improve the anesthetic effect and recovery quality,which is worthy of promotion.
