类风湿关节炎药物及手术治疗的研究进展

Research Progress on Drug and Surgical Treatment of Rheumatoid Arthritis

类风湿关节炎(rheumatoid arthritis,RA)是一种以多关节侵蚀性、对称性发病为主要表现的慢性、全身性自身免疫性疾病,发病机制尚不明确,其治疗方法主要有药物、手术治疗以及其他疗法,其中中医药物治疗主要有含生物碱类的青风藤碱、乌头碱,含苷类的雷公藤多苷和白芍总苷等。西药治疗主要以对症治疗,缓解病情进展,常用的药物有非甾体类抗抗炎药、改善病情抗风湿药、糖皮质质激素、生物制剂等。手术治疗主要用于药物及其他治疗方式效果,且最终出现关节畸形导致功能障碍,手术方式包括传统开放式手术、关节镜下手术、关节置换术、关节腔灌洗术等。就近年来中西医临床治疗RA临常用药物及手术治疗方式进行综述,以期为临床中西医药物及手术治疗RA提供临床参考。

Rheumatoid arthritis(RA)is a chronic,systemic autoimmune disease characterized by erosive and symmetrical multiple joints.Its pathogenesis is still unclear.Its treatment methods mainly include medicine,surgical treatment and other therapies.Among them,traditional Chinese medicine mainly includes alkaloid containing sinomenine and aconitine,glycoside containing tripterygium wilfordii polyglycosides and total paeoniflorin.Western medicine is mainly used for symptomatic treatment to alleviate the progress of the disease.The commonly used drugs are non steroidal anti-inflammatory drugs,anti rheumatic drugs to improve the condition,glucocorticoids,biological agents,etc.Surgical treatment is mainly used for the effect of drugs and other treatment methods,and eventually joint deformity leads to dysfunction.The surgical methods include traditional open surgery,arthroscopic surgery,joint replacement,joint cavity irrigation,etc.This article reviewed the commonly used drugs and surgical treatment methods of traditional Chinese and western medicine in clinical treatment of RA in recent years,with a view to providing clinical reference for traditional Chinese and western medicine and surgical treatment of RA.
