体外发散式冲击波联合火针治疗肩周炎患者的疗效

Effect of the divergent type shock waves combined with fire needle in the treatment of the patients with periarthritis of shoulder

目的观察发散式冲击波联合火针治疗肩周炎患者的临床效果。方法收集2019年4月至2021年4月毕节市第三人民医院康复医学科门诊肩周炎患者50例,其中男32例,女18例,年龄45~65岁,病程3~24个月。所有患者给予冲击波联合火针治疗,间隔4~5 d做1次,4次为1个疗程。观察治疗前,与治疗后1 d、1个月、3个月、6个月VAS与患者Constant-Murley肩关节评分,记录治疗后不良反应和并发症发生情况,比较患者治疗后不同观察时点的疗效。结果与治疗前比较,术后各观察时点的VAS均低于治疗前,Constant-Murley肩关节评分高于治疗前,差异均有统计学意义(P均<0.05)。术后不同观察时点的总有效率分别为:96%(48/50例)、94%(47/50例)、94%(47/50例)、94%(47/50例)。未见出血、神经、肌肉损伤等严重并发症。结论冲击波结合火针治疗肩周炎是一种安全、有效的治疗方法。

Objective To observe the clinical effect of the divergent shock waves combined with fire needle in the treatment of patient with periarthritis of shoulder.Methods Between April 2019 and April 2021,50 patients with periarthritis of shoulder in the Department of Rehabilitation Medicine Clinic,The Third People's Hospital of Bijie City were collected randomly,32 cases of male and 18 cases of female,aged 45 to 65 years,course of 3 to 24 months.All patients were given shock wave with fire needle therapy,1 time interval of 4-5 days,4 times for one course of treatment.Before treatment and after the treatment at 1 day,1 month,6 months,12 months,Constant-Murley shoulder score,VAS of patients were recorded.Results Compared with pretreatment,the VAS was decreased in each observation point after treatment,and Constant-Murley shoulder joint score was higher than that before the treatment,with statistically significant(P<0.05).Postoperative total effective rate of different observation points were 96%(48/50 cases),94%(47/50 cases),94%(47/50 cases),94%(47/50 cases).All patients were successfully completed treatment without serious complications such as bleeding,nerve and muscle damage.Conclusion The shock wave combined with fire needle is a safe and effective treatment in the treatment of periarthritis of shoulder.
