经皮克氏针内固定加石膏托外固定治疗桡骨远端骨折的临床效果分析

Clinical Effect Analysis of Percutaneous Kirschner Wire Internal Fixation Plus Plaster Cast External Fixation for Distal Radius Fracture

目的探讨经皮克氏针内固定加石膏托外固定在桡骨远端骨折临床治疗中的应用效果。方法随机选取漳州市第二中医院2019年9月—2021年9月期间收治的50例桡骨远端骨折患者作为研究对象,实施经皮克氏针内固定加石膏托外固定治疗,观察患者的治疗恢复情况。结果治疗3个月后,患者的尺偏角由(14.17±3.91)°恢复至(24.70±4.37)°,掌倾角由(8.40±1.89)°恢复至(11.51±1.62)°,桡骨缩短长度由(4.71±0.54)mm降低至(1.75±0.82)mm,治疗前后对比,差异有统计学意义(t=2.847、5.209、3.382,P<0.05)。治疗3个月后,患者的Cooney腕关节评分由(67.14±7.33)分提升至(88.25±7.08)分,健康调查简表(SF-36)评分由(50.14±5.52)分提升至(78.32±3.97)分,治疗前后对比,差异有统计学意义(t=3.612、4.533,P<0.05)。随访1~10年,患者的治疗恢复优良率为92.00%。结论在桡骨远端骨折患者的临床治疗中,实施经皮克氏针内固定加石膏托外固定治疗后,能够保障桡骨远端骨折复位效果良好,加快骨伤愈合和腕关节功能恢复,进而提高其生活质量。

Objective To explore the clinical effect analysis of percutaneous Kirschner wire internal fixation plus plas⁃ter cast external fixation for distal radius fracture.Methods Fifty patients with distal radius fractures admitted to the Second Central Hospital of Zhangzhou City from September 2019 to September 2021 were conveniently selected as the study subjects,and the treatment of percutaneous Kirschner wire internal fixation plus plaster cast external fixation was implemented to observe the treatment recovery of patients.Results After 3 months of treatment,the ulnar deviation angle of the patients recovered from(14.17±3.91)°to(24.70±4.37)°,the palmar inclination angle recovered from(8.40±1.89)°to(11.51±1.62)°,and the radial shortening length decreased from(4.71±0.54)mm to(1.75±0.82)mm,and when comparing before and after treatment the difference was statistically significant(t=2.847,5.209,3.382,P<0.05).After 3 months of treatment,the Cooney wrist score of patients with distal radius fracture improved from(67.14±7.33)points to(88.25±7.08)points and the Short Form 36 Questionnaire(SF-36)score improved from(50.14±5.52)points to(78.32±3.97)points,and between before and after treatment the difference was statistically significant(t=3.612,4.533,P<0.05).The excellent treatment recovery rate for patients with distal radius fractures was 92.00%at 1 to 10 years of follow-up.Conclusion In the clinical treatment of patients with distal radius fractures,the implementa⁃tion of percutaneous Kirschner wire internal fixation plus plaster cast external fixation can guarantee good reposition⁃ing of distal radius fractures,accelerate bone healing and functional recovery of the wrist joint,and thus improve their quality of life。
