早期呼吸功能康复训练联合延续康复干预对脊髓损伤患者的影响

Influence of Early Respiratory Function Rehabilitation Training Com⁃bined with Extended Rehabilitation Intervention on Patients with Spinal Cord Injury

摘要 目的分析早期呼吸功能康复训练联合延续康复干预在脊髓损伤患者中的应用效果及对呼吸功能的影响。方法方便选取2020年1月—2021年7月苏州大学附属第一医院收治的脊髓损伤患者78例,以随机摸球法分为两组,各39例。对照组行早期呼吸功能康复训练,观察组行早期呼吸功能康复训练联合延续康复干预。对比两组患者呼吸功能、并发症情况、痰菌阳性率。结果干预前,两组呼吸功能、呼吸肌指标、肺部病菌检测阳性率对比,差异无统计学意义(P>0.05);干预后,观察组患者呼吸功能指标显著高于对照组,差异有统计学意义(P<0.05);干预后,观察组患者吸气肌肌力指数(MIP)、吸气流速峰值(PIF)指标均高于对照组,差异有统计学意义(P<0.05);观察组患者并发症发生率5.13%显著低于对照组25.14%,差异有统计学意义(P<0.05);观察组入院后肺部病菌阳性率2.56%低于对照组20.51%,差异有统计学意义(χ^(2)=4.521,P<0.05)。结论为脊髓损伤患者进行早期呼吸功能康复训练联合延续康复干预可有效提升呼吸功能,降低并发症发生率以及病菌阳性率。 Objective To analyze the application effect of early respiratory function rehabilitation training combined with extended rehabilitation intervention in patients with spinal cord injury and its impact on respiratory function.Methods A total of 78 spinal cord injury patients admitted to the First Affiliated Hospital of Soochow University from January 2020 to July 2021 were conveniently selected,and the random ball touch method was divided into two groups,with 39 cases each.The control group underwent early respiratory rehabilitation training,and the observation group performed early respiratory rehabilitation training and combined continuous rehabilitation intervention.The respiratory function,complications and sputum bacteria were compared between the two groups.Results Before intervention,there were no statistically significant differences in respiratory function,respiratory muscle indexes and positive rate of pul⁃monary bacteria detection between the two groups(P>0.05).After intervention,the respiratory function index of the ob⁃servation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After intervention,the inspiratory muscle strength index(MIP)and peak inspiratory flow rate(PIF)of the ob⁃servation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 5.13%significantly lower than 25.14%in the control group,and the difference was statistically significant(P<0.05).The positive rate of pulmonary bacteria in the observa⁃tion group was 2.56%lower than 20.51%in the control group after admission,and the difference was statistically sig⁃nificant(χ^(2)=4.521,P<0.05).Conclusion Early respiratory function rehabilitation training combined with extended re⁃habilitation intervention for spinal cord injury patients can effectively improve respiratory function,reduce the inci⁃dence of complications and the positive rate of bacteria.
