精细化护理在终末期糖尿病肾病血液透析患者中的应用及对舒适度的影响

Application of Refined Nursing in Hemodialysis Patients with End-stage Diabetic Nephropathy and Its Influence on Comfort

目的探讨终末期糖尿病肾病患者于血液透析治疗期间,采用精细化护理对患者舒适度的影响。方法方便选择台山市人民医院2018年7月—2020年7月行血液透析治疗的终末期糖尿病肾病患者42例,随机分为两组,各21例对照组采用常规护理,研究组在对照组基础上采用精细化护理,比较两组负面情绪、并发症和舒适度。结果护理后,研究组SDS及SAS评分低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率低于对照组,差异有统计学意义(P<0.05);研究组环境舒适(18.42±1.63)分、社会文化舒适(19.55±1.78)分、心理舒适(33.11±3.46)分及生理舒适(28.46±3.22)分高于对照组(10.21±1.21)分、(11.46±1.49)分、(22.74±2.75)分、(18.25±1.86)分,差异有统计学意义(t=18.533、15.971、10.752、12.582,P<0.05)。结论终末期糖尿病肾病患者于血液透析治疗期间采用精细化护理效果显著,能提高患者治疗舒适度和改善负面情绪,降低患者并发症的发生风险。

Objective To investigate the effect of refined nursing on the comfort of patients with end-stage diabetic nephropathy during hemodialysis treatment.Methods Conveniently selected 42 patients with end-stage diabetes nephropathy who underwent hemodialysis in Taishan People′s Hospital from July 2018 to July 2020 were randomly divided into two groups,21 cases in each group.The control group received routine care and the study group received intensive care on the basis of control group.The negative emotions,complications and comfort of the two groups were compared.Results After nursing,the SDS and SAS scores of the study group were lower than those of the control group,the difference was statistically significant(P<0.05).The incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).The environmental comfort(18.42±1.63)points,social and cultural comfort(19.55±1.78)points,psychological comfort(33.11±3.46)points and physiological comfort(28.46±3.22)points in the study group were higher than those in the control group(10.21±1.21)points,(11.46±1.49)points,(22.74±2.75)points,(18.25±1.86)points,the difference was statistically significant(t=18.533,15.971,10.752,12.582,P<0.05).Conclusion The use of refined nursing in patients with end-stage diabetic nephropathy during hemodialysis treatment has a significant effect,which can improve the patient′s treatment comfort,improve negative emotions,and reduce the risk of complications.It should be popularized and applied.
