家庭照顾者照顾能力量表在血液透析患者照顾者中的信效度研究

Reliability and validity of Family Caregiver Task Inventory in hemodialysis patients caregivers

目的 修订家庭照顾者照顾能力量表(FCTI),并分析其在血液透析患者照顾者中应用的信效度。方法 经专家小组评议及预试验调适量表后,采用方便抽样法选取2021年2—12月广州2家三级甲等医院收治的血液透析患者照顾者195例,利用修订版FCTI对其进行调查。采用极端分组检验法(选择总分最高的27%作为高分组,最低的27%作为低分组)和相关分析法评价量表条目,采用Cronbach′s α系数和折半系数评价量表信度,采用已知族群效度、专家评定法、探索性因子分析、验证性因子分析等评价量表效度。结果 高、低分组被测者各条目得分比较,差异均有统计学意义(P<0.05);各条目得分与量表总分的Pearson相关系数为0.193~0.742,经课题组分析讨论后所有条目均保留。量表的总Cronbach′s α系数为0.886,总折半系数为0.715;各维度的Cronbach′s α系数为0.815~0.898,折半系数为0.715~0.966。量表总体内容效度指数为0.800,条目内容效度指数为0.667~1.000。探索性因子分析得到5个公因子,对应量表的5个维度,各条目在主因子的载荷为0.549~0.891。初始模型拟合指标不理想,修正模型各拟合指标处于可接受范围。不同文化程度、经济收入水平照顾者修订版FCTI得分比较,差异均有统计学意义(P<0.05)。结论 修订版FCTI信效度良好,可作为血液透析患者照顾者照顾能力评测工具。

Objective To revise the Family Caregiver Task Inventory(FCTI) and evaluate its reliability and validity in caregivers of hemodialysis patients.Methods FCTI was revised and adjusted by the expert group′s evaluation and pre-experiment.Then a total of 195 caregivers of hemodialysis patients who visited two tertiary hospitals in Guangzhou from February to December 2021 were selected by convenient sampling method, and they were investigated by the revised FCTI.The extreme group test and correlation analysis were used to evaluate the items of the scale, and Cronbach′s α coefficient and half coefficient were used to evaluate the reliability of the scale.The known ethnic validity, expert evaluation method, exploratory factor analysis and confirmatory factor analysis were used to evaluate the validity of the scale.Results The results of item discrimination analysis showed that the mean values of each item were significantly different between the subjects in the high and low score group(P<0.05);the Pearson′s correlation coefficients(r) between the scores of each item and the total score of the scale were 0.193-0.742,and all items were retained after analysis and discussion by the research group.The total Cronbach′s α coefficient of the scale was 0.886,and the total half coefficient was 0.715;the Cronbach′s α coefficient of each dimension was 0.815-0.898;the half coefficient was 0.715-0.966.The overall content validity index of the scale was 0.800,and the item content validity index was 0.667-1.000.Exploratory factor analysis obtained five common factors, corresponding to the five dimensions of the scale, the factor load of each item was 0.549-0.891.In the confirmatory factor analysis, the initial model fitting index was not ideal, and the fitting index of the modified model was in an acceptable range.In the validity analysis of known ethnic groups, the scores of caregivers with different educational levels and economic income levels were compared, and there was statistical significance(P<0.05).Conclusion The FCTI has good reliability and validity, and thus, can be used as a tool to evaluate the caregiver competency of hemodialysis patients.
